Spuriously Elevated Platelet Counts due to Microspherocytosis

Abstract: A patient with an acute burn was noticed to have spuriously elevated platelet counts on an automated system. The major factor accounting for this discrepancy was the presence of numerous microspherocytes which were in the same size range of platelets. It is thus important to add microspherocytosis as an additional cause for a spuriously elevated platelet count. This has special clinical importance when dealing with an acute burn patient.

“…Spuriously high platelet counts determined by automated cell counting were found previously in certain diseases, including microspherocytosis [6], leukemia [7], and hemolytic uremic syndrome [8] as well as in blood salvaged for autotransfusion [2,3]. In the present study the platelet counts obtained by the flow-cytometric analysis were 2.3 to 12 times lower than the figures stated in former studies concerning intra -and postoperative autotransfusion (table 1).…”

“…1 Spurious elevation of the platelet count has been reported when nonplatelet material such as cytoplasmic fragments of RBCs and WBCs are enumerated erroneously as platelets. [2][3][4][5][6][7] Other reports of interference with the platelet count on some of the newer large-volume hematology analyzers include autoagglutinins, cryoglobulins, cold agglutinins, platelet satellitosis, platelet clumps, giant platelets, microcytic RBCs, malaria-infected RBCs, bacteria, and leukemic cell fragments in the tumor lysis syndrome. [8][9][10][11][12] The Cell-DYN 4000 uses optical scatter and electrical impedance to count and size platelets.…”
