Squamous cell carcinoma is a common form of lung cancer, the leading cause of cancer deaths in the world. Identifying early stage lung squamous cell carcinoma patients who would benefit most from effective therapies will reduce the mortality. We have previously shown that microRNAs (miRNAs) were stably present in sputum and potentially useful in diagnosis of lung cancer. The objective of this study was to develop a panel of miRNAs that can be used as a sputum-based test for early stage squamous cell carcinoma of the lungs. This study contained three phases: (1) marker discovery by profiling miRNA expression signatures on 15 lung squamous cell carcinoma and matched normal lung tissue samples with GeneChip miRNA Array; (2) marker optimization by real-time quantitative RT-PCR on sputum of a case-control cohort of 48 stage I lung squamous cell carcinoma patients and 48 healthy individuals; and (3) marker validation on an independent set including 67 lung squamous cell carcinoma patients and 55 healthy subjects. On the surgical tissues, six miRNAs were identified, of which three were overexpressed and three were underexpressed in all 15 tumors. On the sputum samples of the case-control cohort, three (miR-205, miR-210 and miR-708) of the six miRNAs were selected, which in combination produced the best prediction in distinguishing lung squamous cell carcinoma patients from normal subjects with 73% sensitivity and 96% specificity. Validation of the marker panel in the independent populations confirmed the sensitivity and specificity that provided a significant improvement over any single one alone. The sputum markers showed the potential to improve the early detection of lung squamous cell carcinomas.

66Рак легкого в мире на протяжении последних десятилетий остается на первом месте в структуре заболеваемости и смертно-сти у мужчин [1,2].Современная медицина располагает новой аппаратурой и высокотехнологичными методиками диагностики рака легкого. В то же время, при использовании стандартных рентгеноэндо-скопических методов диагностики более 70-80% впервые выяв-ленных больных раком легкого имеют III-IV стадию заболева-ния [2,3]. По данным научно-исследовательских онкологиче-ских центов России, Великобритании и Америки смертность при злокачественных опухолях легкого крайне высокая и достигает 87% от числа заболевших [1][2][3][4].Выделяют две основные клинико-морфологические формы рака легкого, отличающиеся степенью злокачественности, часто-той и характером метастазирования: 1) немелкоклеточный рак легкого (НМРЛ) и 2) мелкоклеточный рак легкого (МРЛ). К НМРЛ относятся плоскоклеточный, крупноклеточный рак и аденокарцинома. Многие авторы и сейчас пользуются этой тер-минологией [2,5,6]. Но в последних публикациях, в том числе морфологической классификации Всемирной организации здра-воохранения (ВОЗ) 2015 г., каждый гистологический тип рассма-тривается отдельно, понятие НМРЛ не используется, хотя оста-ется легитимным [7,8].Считается, что при развитии очага центрального рака легко-го эпителий трахеобронхиального дерева проходит следующие ступени канцерогенеза: трансформацию нормального эпителия в гиперпластический, формирование очагов метаплазии, диспла-зии, рак in situ (CIS) и микроинвазивный рак [9]. в настоящее время в мире рак легкого на протяжении последних десятилетий лидирует в структуре заболеваемости и смертности у мужчин. однако при использовании стандартных рентгеноэндоскопических методов диагностики более 70-80% впервые выявленных больных раком легкого имеют III-Iv стадию заболевания, когда радикальное хирургиче-ское лечение невозможно. развитие центрального рака легкого имеет ступенчатый процесс: трансформация нормального эпителия в гиперпластический, формирование очагов метаплазии, дисплазии, рак in situ (CIs) и микроинвазивный рак. Диагностика рака легкого включает флюорографию, рентгенографию органов грудной клетки, рентгеновскую компью-терную томографию, цитологическое исследование мокроты. однако ведущую роль в диагностике центрального рака легкого играет эндоскопический метод, который благодаря современным методикам (узкоспектральная эндоскопия, ауто-флюоресцентная эндоскопия, увеличительная эндоскопия) позволяет быстро и эффективно выявлять изменения слизи-стой оболочки трахеобронхиального дерева на разных стадиях канцерогенеза.Ключевые слова: рак легкого, эндоскопия, новые технологии диагностики, ранний рак бронхов, эндобронхиальная предопухолевая патология.
