We present a case of epidermoid carcinoma arising in Warthin's tumour of parotid gland in an 80-year-old woman. Light microscopy revealed small foci typical of benign Warthin's tumour with transition to epidermoid carcinoma. Immunohistochemical and ultrastructural observations confirmed the epidermoid features of the malignant cells. The differential diagnosis of a metastasis from an epidermoid carcinoma to the stroma of Warthin's tumour and also of the so-called metaplastic variant of Warthin's tumour is discussed.

BACKGROUND.Despite its well-defined histologic appearance, the often variegated cytomorphologic appearance of Warthin tumor (WT) on fine-needle aspiration (FNA) may lead to an erroneous cytopathologic interpretation. In this study, the authors analyzed the potential sources of diagnostic errors and overall accuracy of FNA diagnosis of WT. 1-3 This is particularly true for salivary gland neoplasms, because FNA diagnosis guides the clinician in decision-making about the management of patients. In the vast majority of patients, careful interpretation of the cytomorphologic findings renders an accurate diagnosis, allowing for conservative management of patients with benign lesions. fourth and seventh decades of life. METHODS.2 WT has a cystic appearance with a double layer of oncocytes surrounding a lymphoid stroma. There are two main cellular components: epithelial and lymphoid. The typical features on cytology include oncocytic cells in cohesive, monolayered sheets; background lymphocytes; and amorphous, cystic debris. 2,4Despite it's well-defined histologic appearance, the often variegated cytomorphology of WT may lead to an erroneous cytopathologic interpretation. The objectives of the current study were to analyze the potential sources of diagnostic errors and examine the overall diagnostic accuracy of FNA for WT from a series of patients at a single institution. MATERIALS AND METHODSA retrospective search of The Johns Hopkins Hospital surgical pathology files (1985)(1986)(1987)(1988)(1989)(1990)(1991)(1992)(1993)(1994)(1995)(1996)(1997)(1998)(1999)(2000)(2001) revealed 97 patients with WT. Of these, 31 patients underwent FNA biopsy. A comprehensive review of the cytopathologic material was undertaken to calculate the overall accuracy and to identify sources of diagnostic error. Smears were reviewed along with the corresponding surgical pathology slides.FNA biopsy was performed by a cytopathologist using a 23-gauge fine needle attached to a 10-mL plastic syringe and employing a Cameco gun. Slides were air dried for Diff-Quik staining, and an on-site, provisional cytopathologic diagnosis was rendered on all slides. Additional smears were prepared and fixed immediately in 95% ethanol for subsequent Papanicolaou staining. In a few cases, needle rinses with balanced salt solution were used to make paraffin cell blocks, and 4-m thin sections were stained with hematoxylin and eosin. On review of the cytologic material, particular attention was paid to identifying the sources of diagnostic errors and correlating them with the histopathologic findings of the resected specimens.
