Squamous cell carcinoma arising in benign adenolymphoma (Warthin's tumor) of the parotid gland

Abstract: Adenolymphoma of the parotid gland is a benign tumor occurring primarily in middle-aged males. Major texts of surgery and pathology state that this tumor is not malignant nor does it undergo malignant degeneration. This report demonstrates squamous cell carcinoma arising in a parotid adenolymphoma in a patient with no other malignancy or previous irradiation to the head and neck.

“…Similarly, the patient in LITTLE & RICKLES' report (22) was shown to suffer from a concomitant squamous carcinoma of the larynx. Another case, published by BAKER et al (23), lacks appropriate documentation; the transition of henign oncocytic into malignant carcinomatous structures presented in their Fig. 3 is not convincing.…”

“…Malignant transformation of the epithelial cofnponent of Warthin's tumour has been reported in 14 cases, as cited by ELLts et al (19). Malignant tutnours arising in Warthin's tutnour inclttde squamous carcinoma (20)(21)(22)(23)(24)(25), undifferentiated carcinoma (14,15), oncocytic adenocarcinoma (11,12,16,26) and rnucoepiderrnoid carcinoma (13). The reports of squamous carcinorna arising in pre-existing Warthin's tumour are not, however, always convincing.…”

“…The squamous component of the lesion must be examined carefully, along with other cellular and background components, to arrive at an accurate diagnosis. It also is prudent to remember that squamous cell carcinoma may coexist with WT; a few published reports have shown that squamous cell carcinoma actually arises from a preexisting WT 16–23…”

“…Moreover, some of the cases published as carcinoma arising in WT have not been considered convincing. Carcinomatous components include squamous cell carcinoma, 7,10,13,15,17,20,25 oncocytic carcinoma, 16,19,21,23 adenocarcinoma, 6,8,18,22,24 undifferentiated carcinoma, 9,12,14 and mucoepidermoid carcinoma (MEC). 11,26,27 Only three cases has been reported so far regarding to MEC.…”
