Squamous cell carcinoma arising in a partially ruptured giant mature cystic teratoma: A case report

Abstract: Mature cystic teratomas are the most common ovarian germ cell tumors and represent 70% of the benign ovarian tumors occurring in women under age 30. In less than 2% of cases, these tumors can transform into malignancies. Squamous cell carcinoma arises most frequently from these tumors (in 80% of cases). Intra-peritoneal rupture accounts for approximately 1%-2% of cystic teratoma complications with most ruptures occurring either intraoperatively during laparoscopic surgery or due to trauma. This case report des… Show more

“… 2 , 15 Our case is a 62-year-old woman with a right-sided tumor which is consistent with other study findings. 3 , 16 , 17 Therefore, it is logical to have a high degree of suspicion of malignant transformation in women aged above 40 years with MCTO. Patients with malignant transformation aged >45 years have worst prognosis ( p < 0.01) than those aged <45 years.…”

“… 18 , 21 CT showing adnexal mass with fat/fluid level and fat droplets would suggest dermoid cyst. 16 However, CT scan and magnetic resonance imaging (MRI) have no role in detecting malignant transformation. In this case, Doppler study was not done as the ovarian mass was not suspected to have malignant transformation.…”

“…Higher levels of CA 125 have been associated with adverse outcomes, but elevated CEA levels are reportedly more useful than either CA 125 or CA 19–9 in the diagnosis of malignant transformation of MCT [ 6 ]. On the other hand, levels of CEA, SCC antigen, CA125 and CA 19–9 are statistically significantly higher in patients with SCC arising from MCT than in those with uncomplicated MCT [ 7 , 8 ]. Furthermore, levels of SCC antigen are elevated on serial monitoring in patients with SCC arising from MCT and in those whose tumors have recurred [ 8 ].…”

“…14 The optimal treatment for MT in cases of MCT is uncertain due to the rarity of the disease. 15 The authors present a case of a 34year-old patient with 14 weeks of gestation who was diagnosed with an adnexal mass suggestive of ovarian teratoma. A laparoscopy salpingo-oophorectomy was performed after 6 months of delivery, and the histological exam revealed a sarcomatoid SCC in the MCT.…”
