Stability in vitiligo: Is there a perfect way to predict it?

Abstract: Stability is a hard-to-define concept in the setting of vitiligo, but is nonetheless extremely crucial to the planning of treatment regimens and also in prognosticating for the patient. There are several ways to judge stability in vitiligo, which include clinical features and, recently, many biochemical, cytological and ultrastructural correlates of the same. These recent advances help in not only in prognosticating individual patients but also in elucidating some of the mechanisms for the pathogenesis of viti… Show more

“…This is coupled with high oxidative stress conditions in the lesional site that alters local skin physiology. Though the clinically unaffected skin appears normal, unpredictable spread of depigmentation indicates substantial instability of peripheral skin 22 23 . Further, it remains unclear if the modified skin biology and localized immune modulation in lesional patches has any association with the resident microflora.…”

“…Previous studies associate vitiligo to autoimmune pathologies due to its frequent association with diseases of this nature, in which melanocyte cell death by immune-mediated apoptosis is considered an important susceptibility factor for vitiligo, emphasized by the detection in serum of auto-antibodies and reactive T lymphocytes targeted against this cell type ( 29 ). In a number of case-control genetic studies of vitiligo, a genetic association of the MHC was observed by testing different MHC markers in patients with different vitiligo phenotypes versus controls ( 30 ).…”

“…Additionally, as far as the overall duration of disease stability is concerned, a consensus still eludes us, with different authors proposing different durations ranging from 3 months to as long as 4 years. [ 19 ] Many attempts have been made over the years to reach to certain conclusions regarding this issue. However, none of these attempts or concepts has stood the test of time.…”
