Introduction: Uptake of neoadjuvant chemotherapy (NC) for muscle invasive bladder cancer (MIBC) has been low despite evidence of a survival benefit. The primary aim of this study was to better understand why the rates are low and determine what factors specifically influence the decision to recommend NC for MIBC. Methods: A 31-question survey was emailed between 2009 and 2011 to medical oncologists belonging to the Canadian Association of Genitourinary Medical Oncologists (CAGMO); and to urologists belonging to the Canadian Urologic Oncology Group (CUOG). We gathered data on practice characteristics, referrals for NC, factors influencing NC use, and chemotherapy regimens offered. Responses were summarized using descriptive statistics. Results: In total, 26/30 (87%) medical oncologists and 25/84 (30%) urologists, who were primarily academic, completed the survey. Most clinicians (medical oncologists 96%, urologists 88%) recommended NC for MIBC, because they considered it to be the standard of care, but most medical oncologists saw ≤6 referrals annually. Performance status, presence of comorbidities and renal function were key considerations in offering NC. NC was not offered if performance status ≥2 (medical oncologists 38%, urologists 44%), age >80 (medical oncologists 46%, urologists 39%), or glomerular filtration rate ≤40 mL/min (medical oncologists 81%, urologists 50%). Conclusions: Most academic clinicians in Canada believe that cisplatin-based combination NC is the standard of care for MIBC and recommend it for patients with adequate performance status and renal function. Using a multidisciplinary approach to treat this disease may be one strategy to increase referral rates for NC and uptake of NC.

Objectives Exposure to chemical weapons and depleted uranium (DU) during multiple wars has a tremendous impact on public health in Iraq. This study conducted to evaluate the regional differences in the effects of these weapons on the histological features of bladder cancer (BC), a cancer form highly associated with environmental risk factors. Patients and MethodsFrom 172 BC patients enrolled in the study 123 were living in the Kurdistan Region (KR) of Iraq and 49 were living in the Rest of Iraq (RI). Medical history, clinical examinations, biochemical investigation and Transurethral Resection of Bladder Tumour (TURBT) or Radical Cystectomy (RC) were performed. Specimens were collected for histopathological examination. SPSS program (V18) used for statistical analysis. Results and DiscussionThe results showed that there are significant differences between the KR and the RI patients in terms of tumour grading and cellular features. Patient samples from the KR patients exhibit high percentage of well differentiated tumours, conversely RI exhibit high percentage of poorly differentiated tumours. Furthermore, histopathological examination showed that majority of tissue samples from KR were transitional cell carcinoma (TCC). The same was true for RI, though previously a higher proportion of tissue samples were squamous cell carcinoma (SCC) in RI. ConclusionThe tumour grading and histopathology showed more advanced cancers in patients living in RI compared to those living in KR. This is because weapons exposure is correlated with cancer aggressiveness of the tumour. Sex differences were obvious as in Iraq only males were participating in military service.

Les tumeurs urothéliales sont classées en fonction de leur grade, degré d'agressivité cytologique et leur stade, niveau d'infiltration de la paroi vésicale. La parution, en avril 2004, du livre de l'OMS a enfin permis la large diffusion de la nouvelle classification du grade des tumeurs vési-cales dite « de consensus » entre l'OMS et l'ISUP qui avait été proposée dès 1998. Cette nouvelle classification insiste par ailleurs sur la description de « variantes morphologiques » des tumeurs urothéliales essentiellement infiltrantes qui se caractérisent par une morphologie différente et une évolution le plus souvent mais non constamment plus péjo-rative. Ces différents caractères que sont le grade et le stade permettent de décider de la prise en charge thérapeutique et de la surveillance ultérieure de ces tumeurs. Les modes d'évaluation de ces critères ont évolué au cours du temps. Mots clés Classification · Grade · Stade · VariantAbstract Urothelial tumors are classified according to their grade, degree of cytological aggression, stage, and level of infiltration of the vesical wall. A new classification system was proposed by WHO/ISUP in 1998 and updated by WHO in April 2004. This new classification also insists on the description of "morphological variants" of essentially infiltrating urothelial tumors that are characterized by different morphology and evolution most often but not consistently more pejorative. The therapeutic treatment and subsequent monitoring of these tumors depend upon the tumor's stage and grade.
