Staged Left Ventricular Recruitment After Single-Ventricle Palliation in Patients With Borderline Left Heart Hypoplasia

Abstract: In these patients with borderline LH disease who underwent SVP, it is possible to increase LH dimensions by using an LV recruitment strategy. In a subset of patients, this strategy allowed establishment of biventricular circulation.

“…Little longitudinal data on the development and progression of fibrosis over time in congenital AS exists, although focal fibrosis in the form of EFE is known to be present prenatally. Surgical techniques have been developed to resect EFE postnatally 18,28 Pharmacologic therapies to modulate the fibrotic pathway are also an area of investigation. 29,30 The effectiveness of these interventions could be non-invasively assessed by measuring longitudinal strain, LGE, and ECV.…”

“…The last variables possibly identify a higher risk anatomic substrate, including borderline LV, associated congenital mitral and aortic arch disease. Whether paucity of alternative strategies justify the risk of the Ross procedure in this infant population or initial single ventricle palliation should be entertained instead remains controversial 19. In addition, infant age per se may be a surrogate for concomitant need for right heart reintervention in children carrying smaller homografts.…”
