Severe angina pectoris in asthma attack: a case report

Asthma is a chronic inflammatory disorder of the airways related to the obstruction of reversible airflow. Asthma presents as recurrent attacks of cough and dyspnea. Poor control causes recurrent admissions to the ICU, and mortality is related to poor drug compliance and follow-up. Angina pectoris is a syndrome of recurrent chest discomfort related to myocardial ischemia. The presence of these two disorders rarely has been reported. We reported a 12-year-old boy who was referred with exacerbation of asthma and developed angina pectoris during hospitalization. He had labored breathing and diffuse wheezing. During treatment of the asthma, the patient developed severe chest pain due to shunt formation and coronary hypoxia, caused by the sole administration of ventolin, since oxygen had been disconnected. After receiving appropriate therapy, both his asthma and angina recovered, and, to date, he has not experienced angina pectoris again.

Hixosfs music is an extension of ext2 Linux filesystem, with additional features to easy accessing and categorizing musical files. Specially it extends the inode struct inside the Virtual file system, considerating as file proprieties meta information such as album, author, title related to the content of a musical file. Comparition have been done respect to the Linux file system in user space Musicmeshfs, the Linux ext2 xattr feature, and ad hoc user space programs for efficiently retriving multimedia data. Since Hixosfs music manages the musical tags at kernel level, it offers higher performances then the other solutions but with less flexibility.

“…In our unit, all COVID-19 ventilated patients with ARDS were evaluated with TEE in accordance with a specific protocol. Although the literature shows that PP in the setting of ARDS has benefits if implemented for at least 18 hours per day, 7 , 8 because of the number of patients who were being simultaneously ventilated and the many difficulties accomplishing this goal, PP was maintained for longer periods, from 48 to 72 hours. In this group of patients who were under deep sedation, receiving neuromuscular blocking agents (NBAs) and requiring vasopressor drugs, hemodynamic evaluation by TEE became crucial, a challenging situation for intensivists.…”

A bstract Background and aim Our aim was to assess the feasibility, safety, and utility of implementing transesophageal echocardiographic screening in patients with coronavirus disease-2019 (COVID-19)-related acute respiratory distress syndrome (ARDS), receiving mechanical ventilation (MV) and in prone position (PP). Methods Prospective observational study performed in an intensive care unit; patients 18 years, with ARDS, invasive MV, in PP were included. A total of 87 patients were included. Results There was no need to change ventilator settings, hemodynamic support, or any difficulties with the insertion of the ultrasonographic probe. Mean duration of transesophageal echocardiography (TEE) was 20 minutes. No displacement of the orotracheal tube, vomiting, or gastrointestinal bleeding was observed. Frequent complication was displacement of the nasogastric tube in 41 (47%) patients. Severe right ventricular (RV) dysfunction was detected in 21 (24%) patients and acute cor pulmonale was diagnosed in 36 (41%) patients. Conclusion Our results show the importance of assessing RV function during the course of severe respiratory distress and the value of TEE for hemodynamic assessment in PP.
