Stem cells in intervertebral disc regeneration–more talk than action?

Abstract: Pain and lifestyle changes are common consequences of intervertebral disc degeneration (IVDD) and affect a large part of the aging population. The stemness of cells is exploited in the field of regenerative medicine as key to treat degenerative diseases. Transplanted cells however often face delivery and survival challenges, especially in tissues with a naturally harsh microniche environment such as the intervertebral disc. Recent interest in the secretome of stem cells, especially cargo protected from microni… Show more

“…Regenerative medicine employs cell-based therapies amidst other strategies ( Kraus et al, 2021 ). Stem or progenitor cells are a valuable but sparse resource and typically require expansion in culture for quality control and enrichment purposes prior to therapeutic applications.…”

“…Several transcripts we found higher in FAT over IVD cells. A benefit of adult stromal cells is their multipotent over pluripotent differentiation potential, a restriction rendering them less tumorigenic than induced pluripotent or embryonic stem cells, however, potentially hampering their plasticity in replacing ailing cells ( Pittenger et al, 1999 ; Kraus et al, 2021 ). Oct4 is known as a stemness and reprogramming marker but also has a role in lineage specification ( Zeineddine et al, 2014 ).…”

“…IVDD can also secrete Stromal cell-derived factor-1a (SDF-1a) to induce the chemotaxis of NPSCs to the NPs center for in situ regeneration (27). IVD regeneration is mainly achieved by inducing the differentiation of MSCs into NPCs or AFCs (28)(29)(30). Although MSCs have achieved good results in bone and cartilage regeneration, similar effects cannot currently be exerted in IVDD, even if the MSCs were delivered to IVDs, due to the lack of vascular systems in the discs (31).…”

“…Given that one of the key elements of the chain of IVD degeneration is a decrease in the number of cells and their metabolic activity, it can be assumed that cell therapy of this pathology should show a fairly high clinical efficacy [5]. In fact, the administration of mesenchymal stem cells (MSCs), which have high regenerative potential and the ability to differentiate in several directions, including chondrogenic, has already given promising results in this pathology [6]. In our previous studies, we have also obtained good outcomes: MSCs contributed to IVD restoration providing the most pronounced stimulatory effect on the fibrochondrocyte density in annulus fibrosus [7].…”
