STEMI mimicker in a 26-year-old man

Abstract: We herein describe a case of acute myocarditis which may mimic myocardial infarction, since affected patients experience ‘typical’ chest pain, the ECG changes are identical to those observed in acute coronary syndromes, and serum markers are increased. This case emphasises the importance of performing appropriate cardiac MRI to help in the differential and definitive diagnosis as well as the extent of myocardial involvement. ST elevation myocardial infarction is rare in young adults and when it is encountered,… Show more

“…As the disease progresses, the sclerotic plaque ruptures in the blood vessel to form thrombus and various embolic substances, block the myocardial blood vessel, affect the blood supply, and then cause myocardial ischemia [ 11 ]. In addition, overwork, low physical activity, overeating, emotional agitation, and advanced age can be used as predisposing factors, which can lead to sudden death of patients [ 12 ]. PCI surgery can perform blood perfusion to the ischemic myocardium, thereby improving the patient's myocardial ischemia, but it can also induce necrosis of myocardial tissues and cause irreversible damage.…”

“…Substance use can have a significant impact on myocardial infarction, particularly in young individuals [ 10 ]. Myocardial infarction, commonly known as a heart attack, occurs when the blood flow to the heart muscle is severely reduced or blocked, leading to damage or death of the heart tissue.…”

“…Myocardial infarction, commonly known as a heart attack, occurs when the blood flow to the heart muscle is severely reduced or blocked, leading to damage or death of the heart tissue. While myocardial infarction is typically associated with older individuals, substance use can increase the risk of heart attack even among the young [ 2 , 10 ].…”

“…Although some case reports have described nonrheumatic streptococcal myocarditis to present as a STEMI mimic [ 12 , 13 ] it is important to recognize that regardless of underlying viral etiology, myocarditis can present similar to STEMI or STEMI-like syndrome and the use of multimodality imaging may aid in diagnosis. Cardiac MRI (CMR) allows for optimal differentiation between normal and diseased myocardial tissue via gadolinium-based contrast agents and has been used as a non-invasive measure for diagnosing myocarditis [ 16 , 17 ]. In acute myocardial infarction (AMI), late gadolinium enhancement (LGE) usually reveals subendocardial or transmural enhancement with edema localized to the culprit vessel whereas in myocarditis the edema may be segmental or diffused [ 16 ].…”
