Background and Aims Visual evoked potentials (VEPs) assess the integrity of the visual pathways from the optic nerve to the occipital cortex. Optic disc cupping and visual field loss have been associated with prolongation of latency of VEP in primary open angle glaucoma (POAG). Methods Pattern reversal and flash VEP tests were done in consenting 20 primary open angle glaucoma eyes and 40 normal control eyes. Results In POAG cases, the refractive error [3.51 ± 1.88 versus 1.88 ± 1.11, D, p = 0.001], cup-disc ratio in percent [66.00 ± 16.98 versus 28.50 ± 5.80, p = 0.001], intraocular pressure [19.55 ± 2.08 versus 11.65 ± 1.64, mmHg, p = 0.001], and automated visual field pattern standard deviation [4.13 ± 6.96 versus 1.64 ± 0.45, dB, p = 0.001] were significantly more than in control. The visual acuity [0.41 ± 0.29 versus 1.00 ± 0.00, p = 0.001], foveal visual sensitivity [25.92 ± 6.88 versus 33.48 ± 1.75, dB, p = 0.001], and automated visual field mean deviation [−9.63 ± 10.58 versus 0.07 ± 1.54, dB, p = 0.001] were significantly less in cases than in control. Among VEP variables, pattern reversal latency N145 [149.00 ± 15.75 versus 137.52 ± 15.20, ms, p = 0.011], flash amplitude N75 [2.18 ± .57 versus 1.47 ± .38, μV, p = 0.001], and flash amplitude N145 [1.99 ± .39 versus 1.43 ± .38, μV, p = 0.001] were increased in cases. The pattern reversal amplitude N75 [1.97 ± .35 versus 2.47 ± .58, μV, p = 0.001], amplitude P100 [3.09 ± .46 versus 6.07 ± 1.44, μV, p = 0.001], and amplitude N145 [2.21 ± .58 versus 4.45 ± 1.99, μV, p = 0.001] were decreased in cases. Conclusions POAG caused glaucomatous damage to optic pathway.

Dynamic random-dot stereograms (dRDS) elicit brain activity generated exclusively by cortical neurons sensitive to binocular horizontal disparity. We studied 20 adults with stereovision deficiency but otherwise normal vision. Psychophysical thresholds were determined with static RDS and with the three-rod experiment. VEP was recorded from seven occipital channels. Stimuli were presented on a monitor by dRDS as stereoscopic checkerboard patterns that moved in depth with 8 depth reversals per second. Horizontal disparity ranged from 7 to 24.5 min of arc. Stimuli were displayed at the center, or in the left or right half field. We determined electrophysiological thresholds as well as the disparity where largest responses occurred. Subjective and electrophysiological thresholds showed a significant positive correlation. In addition, the right visual field was more sensitive to dRDS stimuli than other locations. Squint angle was related to the disparity thresholds. Our data illustrate correlations between clinical symptoms, perceptual deficiency, and VEP parameters.

Objective To systemically explore the range of visual angles that affect visual acuity, and to establish the relationship between the P1 component (peak latency ~100 ms) of the pattern-reversal visual-evoked potential (PRVEP) and the visual acuity at particular visual angles. Methods Two hundred and ten volunteers were divided into seven groups, according to visual acuity as assessed by the standard logarithmic visual acuity chart (SLD-ii). For each group, the PRVEP components were elicited in response to visual angle presentations at 8°, 4°, 2°, 1°/60´, 30´, 15´, and 7.5´, in the whiteblack chess-board reversal mode with a contrast level of 100% at a frequency of 2 Hz. Visual stimuli were presented monocularly, and 200 presentations were averaged for each block of trials. The early and stable component P1 was recorded at the mid-line of the occipital region (oz) and analyzed with SPSS 13.00. Results (1) oz had the maximum P1 amplitude; there was no significant difference between genders or for interocular comparison in normal controls and subjects with optic myopia. (2) The P1 latency decreased slowly below 30´, then increased rapidly. The P1 amplitude initially increased with check size, and was maximal at ~1° and ~30´. (3) The P1 latency in the group with visual acuity ≤0.2 was significantly different at 8°, 15´ and 7.5´, while the amplitude differed at all visual angles, compared with the group with normal vision. Differences in P1 for the groups with 0.5 and 0.6 acuity were only present at visual angles <1°. (4) Regression analysis showed that the P1 latency and amplitude were associated with visual acuity over the full range of visual angles. There was a moderate correlation at visual angles <30´. Regression equations were calculated for the P1 components and visual acuity, based on visual angle. Conclusion (1) Visual angle should be taken into consideration when exploring the function of the visual pathway, especially visual acuity. A visual angle ~60´ might be appropriate when using PRVEP components to evaluate poor vision and to identify malingerers. (2) increased P1 amplitude and decreased P1 latency were associated with increasing visual acuity, and the P1 components displayed a linear correlation with visual acuity, especially in the range of optimal visual angles. Visual acuity can be deduced from P1 based on visual angle.

Abstract: Visual evoked potential elicited by the onset of a random-dot stereogram can be used for objective measurement of stereoacuity in a clinical setting. Differences between controls and glaucomas in high and low disparities could indicate a stereo-specific deficit in glaucoma.
