Sternocleidomastoid pyomyositis

Abstract: Cervical pyomyositis is a rare condition that if not treated appropriately may cause internal jugular vein thrombosis, sepsis and death. Pyomyositis requires a high index of suspicion and should be considered a differential diagnosis in any painful swelling in the head and neck region.

“…Furthermore, GAS infection is a known complication of chicken pox in children [5], although only 1 of the 6 pediatric cases was GAS-positive. Although pyomyositis typically occurs in the lower extremities (9/24, 37.5%) [5, 9–27], other muscle groups can be involved, such as the iliac [10], psoas [11, 17], iliopsoas [11], trunk [5], neck [13, 18, 26, 27], and upper extremity muscles [14, 16, 19, 22, 25]. Involvement of multiple muscle groups is also common (9/24, 37.5%).…”

“…Treatment of pyomyositis consists of administration of antimicrobial agents and drainage of intramuscular abscesses. Empiric antimicrobial choice is primarily targeted to S aureus , which is the most common pathogen . However, because other pathogens such as Streptococci can be involved, a broad coverage of pathogens should be ensured until the actual pathogen is isolated.…”

Drainage of pus can be achieved either by open surgery or by image‐guided needle aspiration .…”
