Subclinical Hypothyroidism in a Biethnic, Urban Community

Abstract: Subclinical hypothyroidism is a common condition in community-living older people, especially women. However, it appeared to have no effect on any of the measures of health status utilized until serum TSH concentrations exceeded 10 microU/mL, and even then the effects were rarely significant.

“…Thus, TSH may be a less sensitive and more indirect measure of actual TH activity and CNS availability, thereby attenuating the relationship between TSH and cognitive performance in euthyroid individuals [19]. As in our study that showed no relationship between tT 3 and cognition, previous studies have also failed to identify a significant relationship between cognitive function and T 3 concentrations [20,21,28]. However, another study supported an association between the variability of T 3 levels within the normal range and cognitive dysfunctions [29,30].…”

“…Additionally, the mere association between SHypo and psychiatric disorders should not lead to the conclusion that SHypo brings about these associated disorders. The four large epidemiological studies that examined cognitive and affective scores in SHypo subjects found few32 33 or no34 35 significant differences overall, and none in those with TSH between 5 mIU/l and 10 mIU/l.…”

“…Nevertheless, taken together, the results suggest that evaluation and treatment of subclinical hypothyroidism in the elderly may be beneficial in view of its prevalence and its potential clinical importance. Indeed, subclinical hypothyroidism is common with estimates ranging from 5% to 17% in the general population, with an age-related increase to as many as 20% of women and 9.5% of men over the age of 60 years [6, 22, 33, 3639]. The prevalence of subclinical hypothyroidism in men over the age of 74 years was 16% which approaches the 21% prevalence seen in women of the same age [40].…”
