Sublobar resection is associated with improved outcomes over radiotherapy in the management of high-risk elderly patients with Stage I non-small cell lung cancer: a systematic review and meta-analysis

Abstract: Background and AimA matched-pair comparison was performed to compare the efficacy and safety of sublobar resection versus radiotherapy for high-risk elderly patients with Stage I non-small cell lung cancer (NSCLC).Patients and MethodsWe searched the Cochrane Library, MEDLINE, CENTRAL, EMBASE and manual searches. The meta-analysis was performed to compare overall survival, pattern of failure, and toxicity among the homogeneous studies. Subdivided analyses were also performed.ResultsSixteen studies containing 11… Show more

“…Since the National Lung Screening Trial approved the use of low-dose computed tomography for lung cancer screening, this method has become popular in the population and an increasing number of early stage non-small cell lung cancer (NSCLC) cases have been diagnosed [ 2 ]. Radical lobectomy is the gold standard for treating early stage lung cancer, although, with the refinement of lung cancer stage definition, sublobar resection has become gradually suitable for partial ≤2-cm lung adenocarcinoma [ 3 6 ]. In addition, sublobar resection is an important tool because it assists in the diagnosis of suspicious malignant pulmonary nodules.…”

“…Although, the current gold standard surgical procedure for NSCLC is still lobectomy (3,4), sublobar resection (SLR) which includes wedge resection and segmentectomy have been reported to be effective by GGOs (ground-glass opacities) (5)(6)(7)(8). Further, SLR is recommended for earlystage NSCLC patients with low tolerance for lobectomy (9)(10)(11).…”

“…7 Surgery has even been shown to demonstrate an increase in survival compared with radiotherapy in this cohort. 8 Age itself is not a contraindication for surgery, nor should it be, although it is understood that it can be a marker of decreased physiological and mental compliance, often proportional to comorbidities. 9 Physiological decline, multimorbidity, and frailty are independent predictors that are associated with aging.…”
