Substituting serum anti-Müllerian hormone for polycystic ovary morphology increases the number of women diagnosed with polycystic ovary syndrome: a community-based cross-sectional study

Abstract: STUDY QUESTION Can serum anti-Müllerian hormone (AMH) replace polycystic ovary morphology (PCOM) determined by ultrasound as a diagnostic component of polycystic ovary syndrome (PCOS)? SUMMARY ANSWER Despite good correlations between serum AMH and PCOM, the use of a high serum AMH as a proxy for PCOM resulted in the reclassification of PCOS in 5% of study participants, with the main effect being more women identified, althoug… Show more

“…We identified a small number of studies that have investigated the value of AMH in the diagnosis of POI, demonstrating a progressive decline in women across the stages of declining ovarian function to POI, although it remained normal in many women until POI was established ( Knauff et al , 2009 ; Li et al , 2011 ; Desongnis et al , 2021 ; Jiao et al , 2021 ). Testing of diagnostic accuracy showed very good discrimination from other causes of oligo/amenorrhoea, notably PCOS, hypogonadotropic hypogonadism and hyperprolactinaemia, where AMH levels are characteristically normal or high ( Li et al , 2011 ; Barbakadze and Kristasashvili, 2014 ; Bradbury et al , 2017 ; Bell et al , 2021a ). Thus, serum AMH may be useful in young women where there is diagnostic uncertainty of the cause of amenorrhoea, with the caveat that AMH can be normal in some women with incipient POI ( Guzel et al , 2017 ).…”

“…INTERIOR COMPONENT Body Inflammation "PCOS" is associated with low-level body inflammation, which can raise the risk of atherogenesis. Inflammation-related atherogenesis may be increased in women with PCOS who are also obsessed, in comparison to healthy women with "PCOS" or obsess controls 35 .…”

“…59 As evident in ►Table 2, different cut-offs have indeed been used to identify PCOM, emphasizing that the cut-off is dependent on the population and the assay used. Consequently, the sensitivity and specificity for PCOM also differ depending on the cut-off used, 50,[52][53][54] and the cut-off will also affect the prevalence of PCOS. 55,56,59 Thus, in future studies, it is important to define how the AMH cut-off has been established.…”
