Purpose:The need for muscular relaxation to improve intubating conditions and to reduce the incidence of laryngeal morbidity is still controversial. The aim of this study was to determine the incidence of symptomatic laryngeal injuries (SLI) and of acceptable intubating conditions (including both good and excellent conditions), both with and without cisatracurium during induction of anesthesia, along with moderate doses of remifentanil and propofol. Methods:In this prospective, randomized double-blind equivalence trial, the intubating conditions were compared in 130 ASA I or II female patients. All subjects received remifentanil 2 µg·kg -1 iv and propofol 2.5 mg·kg -1 iv, with either cisatracurium 0.15 mg·kg -1 iv (group Cisatracturium), or saline (group Placebo). Tracheal intubating conditions were assessed with the Copenhagen Score. A systematic screening for postoperative hoarseness and sore throat was performed 24 and 48 hr after anesthesia, followed by a nasofibroscopic examination when laryngeal symptoms persisted at 48 hr.Results: Twenty-four hr after anesthesia, the incidence of postoperative hoarseness and sore throat in the Cisatracurium and Placebo groups was 26.5% and 21.5%, respectively, and 48 hr after anesthesia, the incidence was 7.8% and 6.1%, respectively (P = 0.32 and P = 0.50 between groups, respectively). In the clinically evaluable population, the incidence of SLI, assessed at 48 hr by nasofibroscopy, was equivalent in both groups, 1.6% vs 1.5% in group Placebo and group Cisatracurium, respectively (P < 0.001 for equivalence test), as was the occurrence of acceptable intubating conditions (95.4% vs 100%, P < 0.05 for equivalence test). However, the occurrence of excellent intubating conditions was more frequent in group Cisatracurium than in group Placebo (P = 0.0003). Conclusion:Following induction of anesthesia with propofol and moderate-dose remifentanil, cisatracurium did not confer a higher rate of good-to-excellent conditions for tracheal intubation, nor did muscle relaxation with cisatracurium decrease the rate of SLI after tracheal intubation.

BackgroundThe Pentax Airway Scope (AWS) is a video laryngoscope designed to facilitate tracheal intubation with a high-resolution image. The Pentax AWS has been reported to cause less hemodynamic stress than the Macintosh laryngoscope. The aims of this study are to investigate the differences in hemodynamic responses and norepinephrine concentrations to tracheal intubation between procedures using he Pentax AWS and the Macintosh laryngoscope.MethodsForty patients (American Society of Anesthesiologists class I-II, age range: 18-60 years) were randomly assigned to be intubated with either the Pentax AWS or the Macintosh laryngoscope while under general anesthesia. Routine monitoring, including invasive arterial blood pressure and bispectral index, were applied. Thiopental (4 mg/kg), fentanyl (1 µg/kg), midazolam (0.05 mg/kg), and rocuronium (0.6 mg/kg) were administered for anesthetic induction. Systolic, diastolic, and mean blood pressures and heart rates were recorded pre-intubation, immediately post-intubation (T0), and over the following 10 minutes at one minute intervals (T1, T2, T3, T4, T5…T10). Patient blood was sampled for norepinephrine concentrations pre-intubation (baseline) and post-intubation (T1). Evidence of sore throat was evaluated 30 min and 24 hr after extubation. Data were transformed to % basal and expressed as mean ± SD.ResultsThe systolic, diastolic, and mean blood pressure, and heart rate at T0 and T4 were significantly different between the two groups. There was no significant difference in plasma norepinephrine between the two groups. The difference in incidence of sore throat was not significant between the two groups.ConclusionsPentax-AWS for tracheal intubation has greater hemodynamic stability than the Macintosh blade laryngoscope.

Sore throat and hoarseness are common complications after endotracheal intubation. The incidence of sore throat and hoarseness after endotracheal intubation varies from 14 to 75%. Although regarded as minor complications, sore throat and hoarseness contribute to postoperative morbidity and patient dissatisfaction. The aim of this study were to determine the incidence of sore throat and hoarseness after endotracheal general anesthesia and to describe associated risk factors at Arifin Achmad Public Hospital. This study used a descriptive cross sectional method. A total of 88 patients who underwent endotracheal general anesthesia were enrolled in this study. Data of the patient such as gender, age, duration of intubation and endotracheal tube size were collected from medical record and direct observation at the time of operation. All of the patients were interviewed within 24-36 hours after surgery to collect data such as smoking history, sore throat complaint and hoarseness complaint. The evaluation of sore throat degree was accomplished with a Capan's scale for sore throat and hoarseness degree was accomplished with a Stout's scale for hoarseness. The result showed that 18 patients (20.4%) experienced sore throat and 34 patients (38.6%) experienced hoarseness after surgery. Pengelolaan jalan napas pasien dalam anestesi dapat dilakukan dengan berbagai cara antara lain dengan manuver tripel jalan napas (triple airway maneuver) atau dengan pemasangan alat seperti alat jalan napas faring (pharyngeal airway), sungkup muka (face mask), sungkup laring (laryngeal mask) dan pipa endotrakeal (endotracheal tube) atau dikenal sebagai intubasi. Di antara berbagai pilihan tersebut, intubasi adalah teknik yang lebih sering digunakan karena lebih cepat, akurat, sederhana dan aman dimana semua sasaran dari pengelolaan jalan napas dapat dicapai yaitu mempertahankan patensi jalan napas, mempermudah oksigenisasi dan mencegah aspirasi. Meskipun memiliki berbagai kelebihan bukan berarti intubasi bebas dari komplikasi. [1][2][3][4][5][6] Nyeri tenggorok dan serak adalah komplikasi intubasi yang paling sering dirasakan pasien setelah pulih dari anestesi. Insidensi nyeri tenggorok dan serak berkisar antara 14-75%. McHardy et al menemukan, dari 1325 pasien yang diintubasi, 50% pasien (663 orang) mengeluhkan timbulnya serak dan 14,4% pasien (191 orang) mengeluhkan timbulnya nyeri tenggorok. Biro et al menemukan, dari 809 pasien yang diintubasi, 40% pasien (323 orang) mengeluhkan timbulnya nyeri tenggorok. Yamanaka et al menemukan, dari 3093 pasien yang diintubasi, 49% pasien (1515 orang) mengeluhkan timbulnya serak. Satriyanto yang meneliti efektivitas ketamin kumur untuk mengurangi nyeri tenggorok pada pasien yang menjalani anestesi umum endotrakeal di RSUP Dr. Hasan Sadikin menemukan 4% pasien pada kelompok ketamin dan 24% pasien
