Sudden bilateral sensorineural hearing loss as the presenting symptom of meningeal carcinomatosis of gastric cancer: Report of a case

Abstract: Meningeal carcinomatosis is the diffuse infiltration of the meninges by metastatic carcinoma. A 62-year-old man presented with a sudden onset of profound hearing loss affecting both ears. Brain magnetic resonance imaging showed increased signal intensity on T1-weighted images of both vestibulocochlear nerves. Upper gastrointestinal endoscopy revealed a large type 3 gastric cancer, and the pathological diagnosis was poorly differentiated adenocarcinoma. Although combination chemotherapy with oral S-1 and weekly… Show more

“…LMC is thought to be relatively rare in gastric cancer, with a frequency of 0.16-0.69% in all gastric cancer patients [4][5][6][7][8]. Peritoneal metastasis is the most frequent and life-threatening mode of metastasis and recurrence in patients with gastric cancer.…”

“…However, although rare, meningeal carcinomatosis can be the initial and early manifestations of the malignancy (as was observed in our patient) [3],[11]–[18]. In these patients, the diagnosis of meningeal carcinomatosis contributed to the identification of the primary malignancy and the primary tumors were found in the stomach [3],[12], the lung [14],[16], the bronchus [15] and the colon [13]. Common neurological presentations are headache [3],[11],[15] and bilateral deafness [12]–[14].…”

“…However, very often, the primary cancer is already known, and therefore facilitates the diagnosis of the metastases 7. Gadolinium-enhanced MRI is accepted as the ‘gold standard’ in investigation of CPA tumours 8.…”
