Superficial Circumflex Iliac Artery Perforator Flap for Dorsalis Pedis Reconstruction

Abstract: Summary:Reconstruction of dorsalis pedis with soft tissue is challenging because it needs to preserve thin structure to ensure that the patient will be able to wear shoes. Here, we report the use of a thin superficial circumflex iliac artery perforator (SCIP) flap in dorsalis pedis reconstruction. A 67-year-old man presented with a third-degree burn, which exposed his extensor tendons. A thin SCIP flap from the left inguinal region measuring 15 × 4 cm was transferred to the dorsalis pedis region. Postoperative… Show more

“…Harvesting such a specific flap on thin patients could be much easier than patients with high BMI. According to our analysis, only 10 articles included information of BMI levels, There are some disadvantages of SCIP flaps, such as having small vessel diameter, 6 limited length of pedicle, 13 and variable locations of the SCIA and related perforators. 14 These attributions do not seem to affect the indications or the recipient areas of this flap.…”

“…5 The well-known disadvantages are the short pedicle, small vessel diameter, and inconsistent anatomy. 6 Initial studies had limited number of cases, mainly describing the anatomy and restricted areas of utilization. Although they give hints about the reliability of the SCIP flap, further indepth investigations of the literature are necessary.…”

“…However, this issue can be properly handled by preoperative Doppler ultrasound examinations. In our practice, we found the ubiquitous existence of only one dominant perforator, the superficial branches and deep branches of the SCIA have a complementary relationship ( 20 ). Patients with a strong and long deep branch of the SCIA generally have a short superficial branch, and a deep branch is considered as the dominant perforator; when patients have short and small deep branches of the SCIA, usually a long and large superficial branch can be identified, in this case, the superficial branch is the dominant perforator.…”

“…Resounding evidence of the benefits of utilizing SCIP in extremities reconstruction was first delineated by Koshima ( 9 ), which paved the way for the repopularization of using SCIP in plastic surgery. The pragmatic advantages of SCIP in defect reconstruction include superficial dissection for the pedicle vessel with reduced flap harvest time, customizable thickness, the obtainability of a large cutaneous vein for an intravenous drainage system, and minimum invasiveness at the donor site ( 20 ). From there forward, SCIP has been widely applied to reconstructions of limbs, penis, ears, and other head and neck defect ( 21 ).…”

“…Il offre une morbidité moindre au site donneur avec une cicatrice dissimulée dans le sous-vêtement. Prélevé fin comme dans notre cas, le lambeau SCIP ne nécessite pas de dégraissage supplémentaire et évite ainsi au patient une nouvelle intervention lors des reconstructions des extrémités distales des membres [9][10][11].…”
