Superior mesenteric artery pseudoaneurysm successfully treated with polytetrafluoroethylene covered stent

Abstract: A postoperative superior mesenteric artery pseudoaneurysm that communicates with a pancreatic pseudocyst after aortic surgery is a difficult management problem. Untreated, this condition can lead to exsanguination. Traditional surgical treatment has many potential complications. Endovascular repair has the potential for avoidance of surgical complications. We present the first superior mesenteric artery pseudoaneurysm successfully treated with A polytetrafluorethylene covered stent.

“…The use of endovascular exclusion of true or false aneurysms with a covered stent has been successfully utilized, as reported in cases of small-caliber peripheral and visceral arteries [16,17]. To our knowledge, there is only one case reported in which a life-threatening hemorrhage of the gastroduodenal artery stump following pylorus-sparing Whipple procedure was successfully treated with a covered stent [8].…”

“…All reports of stent graft exclusion of visceral artery pseudoaneurysms concerned case reports with shortterm follow-up, 13-16 except for two case reports, by Cowan et al 37 and Qu et al, 38 with clinical success at 60 months after implantation without any imaging studies during the follow-up period. To our knowledge, our study is the first series with available midterm assessment of results with clinical and imaging evaluation.…”

“…2,7,8 A few case reports and case series also describe the use of stents and stent grafts to exclude these lesions. 7,[9][10][11][12] In general, these reports (which comprise small numbers of patients) show durable results with duration of follow-up from 3 months to 5 years. [9][10][11][12] One case report described an endoleak at the distal edge of a covered stent at 3 months, 13 which was subsequently treated by thrombin injection.…”
