Supporting Clinical Practice Behavior Change Among Neurologic Physical Therapists

Abstract: Extensive multidimensional KTAs were feasible in inpatient rehabilitation and were accompanied by a moderate increase in documented and self-reported frequency of NSGT attempts. Clinical teams may benefit from adopting KTAs that best support clinical practice change.

“…The search was purposively broad aiming to scope the current literature, and the included studies were published between 1993 and 2020, with 9 of the 12 (75%) studies published in the last five years. The studies were located in North America; USA and Canada [14][15][16][17][18][19] as well as Europe; Sweden, England and the Netherlands [20][21][22][23] and Australia. 24,25 Bravata et al's QI definitions 4 were used for the quality interventions, the included studies evaluated interventions involving Organizational Change, [14][15][16][17]20,22 Provider Education and Audit-Feedback process 19,24,25 and Provider Education.…”

“…Keeping treatment manual information in the public domain is particularly important for stroke rehabilitation interventions because, despite the importance of evidence-based practice, therapists do not identify research evidence as a primary source of information for use in clinical practice. 45 The timing of release of detailed information about an intervention, such as a treatment manual, also needs to be carefully considered. Trialists may wish to delay the release of such information (a) prevent the uptake of an intervention before its effectiveness is known, or (b) to minimize the risk of trial contamination—for example, usual care changing over time as a result of the details of the experimental intervention being in the public domain.…”

“…A recent case study in knowledge translation among neurologic physical therapists provides an example of how the structural environment can be modified to support behavior change related to clinical practice. 19 Patients, their beliefs, health literacy, and engagement in shared decision making 20 can encourage or discourage knowledge translation. Finally, the model suggests that the social structure of a practice environment (politics, personalities, presence of local champions for evidence-based practices, and culture and belief systems) also influences propensity for knowledge translation.…”
