Surgical Complications with the Cochlear Multiple-Channel Intracochlear Implant: Experience at Hannover and Melbourne

Abstract: The surgical complications for the first 153 multiple-channel cochlear implant operations carried out at the Medizinische Hochschule in Hannover and the first 100 operations at the University of Melbourne Clinic, The Royal Victorian Eye and Ear Hospital, are presented. In the Hannover experience the major complications were wound breakdown, wound infection, electrode tie erosion through the external auditory canal, electrode slippage, a persistent increase in tinnitus, and facial nerve stimulation. The inciden… Show more

“…In most of the cases the procedure is technically safe and the audiologic results are satisfactory 5 . Of the thirteen patients who had major complications, the reimplant surgery was done in 4; and from these, 2 were because of device failure and 2 were because of infection.…”

“…Major complications involve meningitis, flap necrosis, device failure, electrode extrusion, facial nerve paralysis and others; while the minor complications involve facial nerve stimulation, electrode migration, vertigo, tinnitus, and others. The major complications which require surgery review and, especially those associated with device insertion are not common 5 . In a study carried out in Latin America involving 40 cochlear implant centers in 10 countries, with a sample of 3,768 implanted patients, there were 193 postoperative complications (5.1%), and the major causes were: spontaneous system failure (86 cases), skin inflammation caused by the magnet (35 cases) and infection (26 cases).…”

“…Although the prevalence of complications in our sample is higher than those from most studies in the literature, we believe this can be explained by the fact that we operate cases of higher complexity, with cochlear ossification and inner ear malformation [8][9][10][11][12][13][14][15][16] . Moreover, our clinic is a center for the training of ear surgeons, with a complementary specialization program, in which trainee surgeons participate actively in the cochlear implant surgeries.…”
