Introduction Carotid atherosclerotic disease is a known independent risk factor of post operative stroke after coronary artery bypass grafting (CABG). The best management of concomitant coronary artery disease and carotid artery disease remains debated. Current strategies include simultaneous carotid endoarterectomy (CEA) and CABG, staged CEA followed by CABG, staged CABG followed by CEA, staged transfemoral carotid artery stenting (TF-CAS) followed by CABG, simultaneous TF-CAS and CABG and transcarotid artery stenting. Methods We report our experience based on a cohort of 222 patients undergoing combined CEA and CABG surgery who come to our observation from 2004 to 2020. All patients with >70% carotid stenosis and severe multivessel or common truncal coronary artery disease underwent combined CEA and CABG surgery at our instituion. 30% of patients had previously remote neurological symptoms or a cerebral CT-scan with ischemic lesions. Patients with carotid stenosis >70%, either asymptomatic or symptomatic, underwent CT-scan without contrast media to assess ischemic brain injury, and in some cases, if necessary, CT-angiography of the neck and intracranial vessels. Results The overall perioperative mortality rate was 4.1% (9/222 patients). Two patients (.9%) had periprocedural ipsilateral transient ischemic attack (TIA) which completely resolved by the second postoperative day. Two patients (.9%) had an ipsilateral stroke, while 7 patients (3.2%) had a stroke of the controlateral brain hemisphere. Two patients (.9%) patients were affected by periprocedural coma caused by cerebral hypoperfusion due to perioperative heart failure. There were no statistically significant differences between patients in Extracorporeal Circulation (ECC) and Off-pump patients in the onset of perioperative stroke. Conclusion Our experience reported that combined surgical treatment of CEA and CABG, possibly Off-Pump, is a feasible treatment procedure, able to minimize the risk of post-operative stroke and cognitive deficits.

Тестирование и оценка эффективности работы нового интерактивного калькулятора для стратификации хирургического риска и выбора оптимальной тактики реваскуляризации у пациентов с сочетанным поражением коронарных (КА) и брахиоцефальных артерий (БЦА). Материал и методы. По результатам выполненных в НИИ Комплексных проблем сердечно-сосудистых заболеваний исследований разработана математическая модель персонифицированного выбора оптимальной тактики реваскуляризации миокарда и головного мозга, основанная на прогнозировании риска развития ишемических сердечно-сосудистых осложнений (ССО) в отдаленном периоде наблюдения (на протяжении 33,95±12,05 мес) при реализации четырех основных хирургических тактик. Работа математической модели протестирована ретроспективно на выборке больных, ранее не включенных в разработку модели. В течение года (июнь 2015 г.-июнь 2016 г.) выполнено 128 операций у пациентов с сочетанным поражением КА и БЦА, в их числе 61 (47,6%) одномоментная операция каротидной эндартерэктомии (КЭЭ и коронарного шунтирования (КШ); 35 (27,3%) гибридных операций чрескожного коронарного вмешательства (ЧКВ) + КЭЭ; 25 (19,5%) поэтапных операций КШ-КЭЭ и 7 (5,5%) поэтапных вмешательств КЭЭ-КШ со средними сроками между этапами реваскуляризации 27,4±15,9 дня. Результаты. Математическая модель отдавала преимущества таким тактикам реваскуляризации, как КЭЭ+КШ и поэтапное КШ-КЭЭ. Данная тенденция объясняется, прежде всего, особенностями выборки больных, которые характеризовались наличием клинически проявляющегося гемодинамически значимого мультифокального атеросклероза, а также выраженной сопутствующей патологией, что обусловливает необходимость применения комбинированного хирургического подхода, одноэтапного или поэтапного с ограниченным 30 днями интервалом времени между операциями. Представляет интерес, что согласно расчетам автоматизированной системы в 79% случаев у пациентов, впоследствии имевших ССО, следовало применять одну из таких тактик, как КЭЭ+КШ и поэтапное КШ-КЭЭ, тогда как выбор мультидисциплинарной комиссии избирал эти две тактики лишь в 57% случаев. Заключение. Настоящий интерактивный калькулятор может занять определенное место в арсенале методов стратификации хирургического риска и выбора тактики реваскуляризации, основанной на прогнозировании ССО в отдаленном периоде наблюдения.

Introduction:Integrity of the great saphenous vein (GSV) endothelium is the most important key element for long-term patency rate of grafts in coronary artery bypass graft (CABG). Storage solutions play an important role in maintaining viability of vein endothelium. Diminished nitric oxide (NO) because of endothelial dysfunction may facilitate vascular inflammation and formation of atherosclerotic plaque.Aim:So, we decided to find a reasonable alternative preservative solution instead of heparinized blood (HB) by measuring NO concentration with Griess assay.Material and Method:SVG samples were obtained from 54 patients undergoing elective CABG. 3 mm rings were stored in solutions: heparinized blood (HB), Krebs (K), Krebs + Propranolol (K+P) 6.66 g/l, Krebs + Adrenaline (K+A) 200 µl/l, and Krebs + Verapamil (K+V) 200 µl/l for 30, 45, 60 and 90 min. Nitrite concentration was measured by Griess assay at 540 nm. H&E staining was performed for histologic test. Statistical analysis was performed using SPSS (V16). Results were expressed as (Means ± SE) followed by One-Way ANOVA for finding best preservative solution. Repeated measurement test was used to investigate best time. In all analysis, (P<0.05) was considered significant.Results:Average concentration of NO in (K+V) compare with HB (1st control), K (2nd control), (K+A) and (K+P) showed higher rate in all times from 30 to 90 min (16.55±1.85:) and in (K+A, K+P) compare with (HB) and (K) there was no statistically significant difference in the same times. Comparing the average concentration of (NO) between (HB) and (K) showed no significant difference (K+V>HB=K=K+A=K+P). Also, our investigations showed that NO concentration in (K+V) has the highest rate in time 90 min (10.07±0.56, p=0.002):. More than 50 percent of endothelial cells stay normal in (K+V) compare with other solutions.Conclusion:It seems that (K+V) is the best solution for the maintenance of normal physiology of SVGs endothelial cells. The most appropriate SVGs endothelial function is within 90 minutes after harvesting.
