Osteoarthritis (OA) is a disease with a high negative impact on patient’s quality of life and a high financial burden. It is a source of chronic pain and affects all mammals, including humans and dogs. As the dog is a common model for translation research of human OA, and exploring spontaneous dog OA can improve the health and well-being of both humans and dogs. To describe the effect of the intra-articular administration of stanozolol in a naturally occurring canine OA model, forty canine (N = 40) hip joints were randomly assigned to receive stanozolol or saline (control). On treatment day and at 8, 15, 30, 90, and 180 days post-treatment, several evaluations were conducted: weight distribution, joint range of motion, thigh girth, digital thermography, and radiographic signs. Also, synovial fluid C-reactive protein and interleukin-1 levels were evaluated. Results from four Clinical Metrology Instruments was also gathered. Results were compared with Repeated Measures ANOVA, with a Huynh–Feldt correction, paired-samples t-test, or Wilcoxon signed-rank test, with p < 0.05. OA was graded as mild (90%), moderate (5%), and severe (5%), including both sexes. They had a mean age of 6.5 ± 2.4 years and a bodyweight of 26.7 ± 5.2 kg. No differences were found between groups at treatment day in all considered evaluations. Weight distribution showed significant improvements with stanozolol from 15 days (p < 0.05) up to 180 days (p < 0.01). Lower values during thermographic evaluation in both views taken and improved joint extension at 90 (p = 0.02) and 180 days (p < 0.01) were observed. Pain and function scores improved up to 180 days. In the control group, radiographic signs progressed, in contrast with stanozolol. The use of stanozolol was safe and produced significant improvements in weight-bearing, pain score, and clinical evaluations in a naturally occurring canine OA model.

Background: Osteoarthritis (OA) is a chronic protracted inflammatory joint disorder that frequently coexists with other comorbidities. Previous studies analyzed peptidase activity in synovial fluid in knee OA (KOA) patients, but they did not investigate whether type of therapy influences enzyme activity. Moreover, it is yet unknown if the activity of any particular enzyme can be taken into account as a reliable prognostic marker for the severity and progression of KOA. Objective: The aim of the current study is to assess synovial fluid peptidase's potential role as a biomarker for KOA in Egyptian patients as well as to determine whether it can predict the clinical progression of such disease. Patients and methods: A cross-sectional study was estimated on 75 KOA patients (Ahlback grade 3 or higher and indicated for arthrocentesis), who were subjected to sociodemographic and clinical characteristics, as well as Functional and Physical Health Status assessed every month. Puromycin sensitive aminopeptidase (PSA), neutral aminopeptidase (NAP), prolyl endopeptidase (PEP), aminopeptidase B (APB) glutamyl aminopeptidase (GLU), aspartate aminopeptidase (ASP), and pyro glutamyl aminopeptidase (PGAP) are among the peptidases shortlisted for their activity as biomarkers in synovial fluid. Results: A significant association between NAP (P=0.038) and APB (P=0.05) peptidases and pain generation on movement in patients with KOA was noticed. Only ASP peptidase showed significant correlation with range of motion (P=0.049). Another clinical condition of knee failure significantly influenced by the presence of ABP (P=0.013) and PEP (P=0.033) peptidase presence. When diagnosing knee effusion, PEP (P=0.050) and PGAP (P=0.018) differ significantly depending on the clinical symptom. PSA was protective (P=0.003), whereas GLU and PEP were risk factors, according to a binary logistic regression (P values 0.001 and 0.019, respectively). Conclusion: Peptidase activity is considered an important synovial fluid biomarker to determine disease progression in osteoarthritic patients, which could aid in early disease detection and the development of better treatment protocols.

BackgroundSynovial macrophages play important roles in the formation and progression of osteoarthritis (OA). This study aimed to explore the biological and clinical significance of macrophage-associated genes (MAGs) in OA.MethodsThe OA synovial gene expression profiles GSE89408 and GSE82107 were obtained from the GEO database. Single-sample gene set enrichment analysis (ssGSEA) and GSEA were employed to decipher differences in immune infiltration and macrophage-associated biological pathways, respectively. Protein–protein interaction (PPI) network analysis and machine learning were utilized to establish a macrophage-associated gene diagnostic signature (MAGDS). RT-qPCR was performed to test the expression of key MAGs in murine models.ResultsOA synovium presented high levels of immune infiltration and activation of macrophage-associated biological pathways. A total of 55 differentially expressed MAGs were identified. Using PPI analysis and machine learning, a MAGDS consisting of IL1B, C5AR1, FCGR2B, IL10, IL6, and TYROBP was established for OA diagnosis (AUC = 0.910) and molecular pathological evaluation. Patients with high MAGDS scores may possess higher levels of immune infiltration and expression of matrix metalloproteinases (MMPs), implying poor biological alterations. The diagnostic value of MAGDS was also validated in an external cohort (AUC = 0.886). The expression of key MAGs was validated in a murine model using RT-qPCR. Additionally, a competitive endogenous RNA network was constructed to reveal the potential posttranscriptional regulatory mechanisms.ConclusionsWe developed and validated a MAGDS model with the ability to accurately diagnose and characterize biological alterations in OA. The six key MAGs may also be latent targets for immunoregulatory therapy.
