Purpose This study aimed to evaluate the diagnostic value of the different Complete blood count-derived systemic inflammation indexes, including neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), systemic immune-inflammation index (SII), and neutrophil to lymphocyte × platelet ratio (NLPR) in differential diagnosing the chronic calculus cholecystitis (CCC) and the acute calculus cholecystitis (ACC), as well as determining the severity of ACC. Patients and Methods 105 CCC and 88 ACC patients were enrolled. NLR, PLR, SII, and NLPR were evaluated in both cohorts as well as in different severity levels of ACC. The severity of ACC was determined based on the Tokyo Guidelines. Receiver operating characteristic (ROC) curve and Univariate/multivariate regression analyses were conducted. Results The levels of NLR, PLR, SII, and NLPR were significantly higher in the ACC group compared to the CCC group. The optimal cutoff values for NLR, PLR, SII, and NLPR were determined to be 3.89, 144.7, 896.8, and 0.031 respectively. NLR>3.89 demonstrates the highest predictive capability with an AUC of 0.801 and a sensitivity of 72.73%. Multivariate analysis showed that NLR>3.89 (OR: 4.169, p = 0.004) and NLPR>0.031 (OR: 4.304, p = 0.005) were dominant in distinguishing ACC from CCC. In ACC patients, the levels of NLR, SII, and NLPR were significantly higher in the Moderate to Severe-degree ACC (MS-ACC) group than in Mild-Degree ACC (M-ACC). NLPR > 0.044 exhibited the highest predictive ability with an AUC of 0.778 and a specificity of 91.67%. Multivariate analysis showed that NLR>6.399 (OR: 10.308, p = 0.000) was a possible independent prognostic factor for accessing the severity of ACC. Conclusion Systemic inflammation indexes can be useful in predicting the risk of ACC and MS-ACC. NLR demonstrates the best distinguishing power and sensitivity for distinguishing ACC from CCC, while NLPR shows the best predictive power and specificity for predicting the severity of ACC.

Introduction Heart failure (HF) is a structural and functional disease that affects millions of people worldwide. The role of inflammation has been demonstrated in many diseases. However, the impact of the Systemic Inflammatory Response Index (SIRI) and the Systemic Immuno-Inflammation Index (SII) on mortality in HF patients has not been adequately investigated. Therefore, in this study, we aimed to demonstrate the effect of SIRI and SII on mortality in patients diagnosed with HF. Methods Our research is a retrospective, single-centre study comprising patients who were diagnosed with HF and presented at the emergency department. We focused on those diagnosed with decompensated HF between January and November 2022. The study recorded the demographic information and hemogram parameters of the patients. The patients’ in-hospital mortality status was recorded and the effect of these parameters on mortality was assessed. Results We recruited 122 eligible patients for our research study. Patients with mortality exhibited significantly higher median SIRI levels compared to those without mortality (6.07 (1.99-12.23) vs 2.46, p=0.038), and the group with mortality had significantly higher median SII levels compared to the other group (6625.05 (4704.73-7539.51) vs. 982.48 (180.69-1929.45); p<0.001). ROC curves were generated to assess the efficacy of WBC, SIRI, SII, and CRP parameters in discriminating mortality, and it was confirmed that WBC, SIRI, and SII were all statistically significant predictors of mortality. SIRI and SII demonstrated superior diagnostic ability compared with WBC and CRP, as evidenced by their respective AUC values of 0.929 and 0.671, sensitivities of 91.7% and 50%, and specificities of 91% and 94%. Conclusions SIRI and SII can be used as mortality indicators in heart failure patients.

Introduction:The 2018 Tokyo Guidelines are insufficient for preoperative prediction of severe acute cholecystitis (AC) preoperatively. The Parkland Grading Scale (PGS) aids in intraoperative assessment but lacks preoperative utility. Biomarkers like C-reactive protein/ albumin ratio (CAR) and systemic immune-inflammation index (SII) were explored for preoperative prediction of severe AC. The present study aimed to investigate the efficacy of biomarkers against severe AC. Methods: A retrospective analysis at the University of Health Sciences Turkey, Fatih Sultan Mehmet Training and Research Hospital covered patients undergoing early laparoscopic cholecystectomy for AC from January 2014 and January 2023. AC was defined according to the 2018 Tokyo Guidelines criteria. Patients were categorized into two groups based on the PGS for intraoperative findings: Group 1 (grades 1-3) for mild AC and group 2 (grades 4-5) for severe AC. Clinical parameters, intraoperative findings, postoperative outcomes, and biomarkers, including CAR and SII, were analyzed. Results: Of 141 patients, 93 were included in group 1 and 48 in group 2. Group 2 exhibited longer operation times, higher rates of conversion to open cholecystectomy, and complications, and prolonged hospital stays. Clinical parameters such as age, sex, symptom duration, and ASA score were varied between the groups. Biomarkers including C-reactive protein and white blood cell count differed significantly between the groups, with CAR and SII identified as predictive factors for severe AC. The cut-off points were 1.86 for CAR and 1327.69 for SII. Conclusion: Preoperative biomarkers, particularly CAR and SII, can effectively predict severe AC. Levels exceeding 1.86 for CAR and 1327.69 for SII indicate increased conversion to open cholecystectomy and postoperative complication risk.
