Systemic sclerosis in pregnancy

Abstract: Systemic sclerosis is a rare multisystem connective tissue disease. It predominantly affects women and poses a significant risk to mother and baby during pregnancy if not managed appropriately. The commonest manifestations are skin fibrosis and Raynaud’s phenomenon. Subgroups of women have an increased risk of organ involvement, especially interstitial lung disease, pulmonary arterial hypertension and renal crises. Pregnancy increases the risk to the mother, especially those with established organ involvement,… Show more

“…Other serious complications of pregnancy such as pre-eclampsia and HELLP (haemolysis, elevated liver enzymes, low platelet) syndrome can mimic SRC and renal biopsy may be required to differentiate the processes. Renal biopsy carries close to the same risk as the general population early in pregnancy but should be avoided in the third trimester unless it will determine management as the risk of complications increases with gestational age [ 68 ]. An ACEi should be started immediately if SRC suspected [ 68 , 70 ].…”

“…The primary concern in pregnancy is presence of PAH, which is the leading cause of maternal mortality. 58 Those considering pregnancy should be evaluated for PAH as part of monitoring and counselled when considering pregnancy. In cases of established PAH, patients should be strongly advised against pregnancy.…”

“…Of note, EULAR recommendations currently advise to avoid pregnancy in SSc patients with PAH as this complication is associated with a significant pregnancy risk and a maternal mortality up to one third of cases. 60 Regardless of the concomitant treatment with teratogenic medications, contraceptives should be prescribed in SSc-PAH patients, if not contraindicated.…”
