T helper 2 inflammatory markers are associated with recurrence in chronic rhinosinusitis with nasal polyps after endoscopic sinus surgery

Abstract: Background: Despite maximum medical treatment and endoscopic sinus surgery (ESS), chronic rhinosinusitis with nasal polyps (CRSwNP) can require revision surgery. With a growing literature on the diversity of cytokine inflammation patterns in CRSwNP, an endotype-driven approach could lead to the identification of cytokine profiles that predict recurrence. Methods: A monocentric longitudinal study was carried out until June 2019 following CRSwNP patients who underwent surgery for the first time between December … Show more

“…Evidence of type 2 inflammation is one criterion for initiating treatment with the biologics currently available for CRSwNP ( statement 22 ) [ 1 , 10 ]. However, strategies that allow more accurate identification of patients eligible for biologics, and specific biomarkers and tools for assessing response to biologics, are currently lacking and are the subjects of intense research [ 10 , 54 , 55 , 74 , 76 , 77 , 78 , 79 ]. What constitutes “clear evidence” of type 2 inflammation is yet to be determined ( see statement 8 ).…”

“…The relationship between serum total IgE levels and the recurrence of nasal polyps remains inconclusive. [40][41][42] Some biotherapeutics, such as monoclonal antibodies against IgE, have been proposed as novel treatments for severe nasal polyps. 43 The association between serum IgE and the treatment response of nasal polyps in varied endotypes deserves further investigation.…”

“…The impact of AD on the recurrence of nasal polyps probably stemmed from its higher ECP level, so it was not significant in the multivariate regression analysis. The relationship between serum total IgE levels and the recurrence of nasal polyps remains inconclusive 40‐42 . Some biotherapeutics, such as monoclonal antibodies against IgE, have been proposed as novel treatments for severe nasal polyps 43 .…”

“…Total (% of total) 441 (100%) 151 (34.0%) 39 (8.2%) that would go on to receive revision surgery will give policymakers an indicator of the number of eligible patients when deciding on a national implementation strategy for biological treatment for CRSwNP. We base the assumption of these patients being a surrogate for eligible patients on the generally accepted fact that patients with CRSwNP are characterised by type 2 inflammation (17,18) and that high rates of verified comorbid asthma as high as 65% have been reported in operated patients (19) . Moreover, with a prevalence of 74-100% in re-operated patients when tested with more sophisticated tests, loss of smell is described as a major symptom, with between 76-98% of reoperated patients having anosmia (20)(21)(22) .…”
