Tertiary Hyperparathyroidism With Renal Osteodystrophy on 18F-Fluorocholine PET/CT

Abstract: A 27-year-old man, with a history of chronic renal failure due to obstructive uropathy, treated with hemodialysis and awaiting kidney transplantation, presented with tertiary hyperparathyroidism. Despite regulation of phosphate and calcium and treatment with cinacalcet, the serum parathyroid hormone levels remained elevated. The patient was referred for parathyroidectomy, and a F-fluorocholine PET/CT was acquired for preoperative planning. Besides visualization of the hyperfunctioning parathyroid glands, the s… Show more

“…18 F-choline-avid degenerative changes were observed in four patients, a 18 F-choline-avid osteoid osteoma in one patient and vertebral hemangiomas with decreased 18 F-choline uptake in three patients. In two patients, both diagnosed with tertiary hyperparathyroidism, a diffusely increased uptake of 18 F-choline was noticed in the skeleton, accompanied with diffuse sclerosis on CT. 8 …”

“…18 F-NaF PET/CT imaging of parathyroid adenoma with hyperparathyroidism and osteitis fibrosa cystica has been reported [ 6 ]. Moreover, visualization of ROD has been described with the use of 18 F-Fluorocholine, a tracer for parathyroid imaging [ 7 ]. To our knowledge, the simultaneous combination of 18 F-Fluorocholine PET/CT imaging and 18 F-NaF PET/CT whole-body bone imaging in the diagnosis of high turnover ROD due to SHPT has not been reported previously.…”

“…In recent years, many studies have been performed on the use of 18 F-FCH PET/CT in primary hyperparathyroidism with promising results [ 7 , 8 ]. However, its performance in secondary and tertiary hyperparathyroidism is limited [ 9 , 10 ]. Our report showed how nuclear medicine imaging can provide effective and complete diagnostic information, essential for HPT patient management.…”
