Tessier 30 facial cleft with duplication of tongue

Abstract: A case of midline cleft of the lower lip, mandible, and the neck with complete duplication of the tongue repaired at neonatal period is reported here. Median cleft of the lower lip, mandible, and bifid tongue with ankyloglossia is reported in the literature, but cleft of the neck with complete duplication of the tongue as a part of the Tessier 30 cleft is very rare. We could not find such report in the available English literature.

“…Midline clefts of the lower lip, tongue, and mandible are a rare type of facial cleft classified as “Tessier 30.” They are associated with a midline lower lip cleft, cleft of the mandibular symphysis, bifid tongue, and ankyloglossia and can continue to the manubrium sterni and involve hyoid bone, thyroid cartilage, and strap muscles of the neck (Millard et al, 1971; Goswami, 2017). These clefts are often associated with dental anomalies and soft tissue skin tags and can be markedly asymmetrical (Tafreshi et al, 2015).…”

“…Most of the cases reviewed by Freitas et al involved a two-staged surgery and the techniques did not vary significantly; the general approach to treatment included a release of the tongue, excision of the clefts and primary closure of the defects. 3 Duplication of the tongue can be corrected using a wedge excision or Z-plasty 11 and this Z-plasty technique can also be utilized if the defect extended on the neck. 3 In our case, glossoplasty, cheiloplasty and mentoplasty served as the first stage of reconstruction in anticipation to the functional deficits caused by a Tessier 30 cleft.…”
