BackgroundAnkle sprain reduces capacity for neurosensory information processing, and these patients commonly progress to chronic ankle instability (CAI). To address this problem, the short-foot exercise (SFE) may be used. However, there has been no previous research on the neurosensory impact of SFE.Therefore, the aim of this study was to assess improvement of quantitative neurosensory indicators after SFE and to determine the effect of proprioceptive sensory exercise (PSE) in patients with CAI.Material/MethodsThe present study included 30 adults (age range: 19–29 years; 50% female). Selection criteria for CAI (Cumberland Ankle Instability Tool ≤24) were implemented, and participants were randomly allocated to 2 groups: SFE (n1=15) and PSE (n2=15). Exercises were conducted 3 times per week for 8 weeks. Measurements of quantitative somatosensory of joint position sense and vibration sensory thresholds, dynamic balance, and ankle instability assessment were evaluated before and after completion of each intervention. Data were analyzed using a repeated- measures analysis of variance.ResultsIn a time-by-group comparison, the SFE group showed a more significant improvement with regards to eversion joint position sense (F1,28=4.543, p<0.05). For vibration sensory threshold, the SFE group showed a more significant improvement (F1,28=8.280, p<0.01). Balance index according to anterio-posterior, mediolateral, and overall index the SFE group a more significant improvement (F1,28=6.666, 4.585, 5.207, p<0.05). And ankle instability SFE group showed a more significant improvement (F1,28=6.890, p<0.05).ConclusionsSFE is more effective than PSE for treating ankle sprain patients. There is a need to develop and promote an effective and controlled exercise program to facilitate the return of ankle sprain patients to normal daily life.

Tibiotalar arthrodesis is commonly used to treat end-stage ankle osteoarthritis. Post-operative impairments are often attributed to limited ankle motion. However, whether muscular deficits also exist, thereby potentially contributing to impairments, is unknown. This study aimed to identify post-operative deficits in ankle musculature by examining range of motion, strength (maximum isometric ankle joint torque), and leg composition (cross-sectional area of individual tissue types: bone, subcutaneous adipose, intramuscular adipose, muscle). Ten individuals with unilateral tibiotalar arthrodesis participated. Paired t-tests (p < 0.05) identified differences between the fused and contralateral, control limb. The results indicate that individuals with tibiotalar arthrodesis have profound losses of range of motion and strength. Across participants, range of motion in the fused limb was 53.5 AE 11.7%, 66.8 AE 6.3%, 38.2 AE 18.7%, 37.8 AE 13.6% less than the control for dorsiflexion, plantarflexion, inversion, and eversion, respectively. The largest strength deficit was in dorsiflexion, with the fused limb producing 47.2 AE 9.4% less torque than the control. The quantity and quality of muscle tissue was also negatively affected in individuals following tibiotalar arthrodesis. The total cross-sectional area of the fused limb was 11.4 AE 5.4% smaller than the control limb. This change was primarily due to the 16.1 AE 6.7% decrease in muscle cross-sectional area. However, intramuscular adipose was significantly increased. Although the posterior compartment demonstrated a significant decrease in cross-sectional area, when accounting for differences in total cross-sectional area, the relative sizes of the four muscle compartments were consistent between limbs. This cross-sectional study motivates longitudinal research examining muscular deficits and whether these deficits are reversible with exercise and rehabilitation. ß

The perception and execution of musculoskeletal control and movement are mediated primarily by the central nervous system and involve the integration of three main subsystems: somatosensory, vestibular, and visual. Balance performance and its measurement are influenced by these sub-systems. This present study was carried out to investigate the effectiveness of neuromuscular-controlled training exercises, comprising BOSU-ball balance training and conventional physiotherapy exercise training protocols, in terms of stability, balance, and proprioception in athletes diagnosed with lateral ligament injury of the ankle joint. The Y Balance test was used as a reliable and valid tool for quantitative balance assessment. Thirteen players (aged between 19 to 26 years, Mean age = 23.2; SD = 2.46) suffering from grade II & grade III lateral ligament injury were recruited from the OPD of the department of Orthopaedics of the Hospital of Universiti Sains Malaysia. All participants were subjected to pre-intervention or baseline assessments consisting of a of Y-balance test and assessment of proprioception employing Biodex 4 Isokinetic Equipment. Participants were then introduced to 12 sessions of a neuromuscular controlled training program (30 min/day; 2 days/wk. for 6 wk.). Findings of the study revealed that six weeks of neuromuscular exercises training significantly improved balance, with certain significant aspects of proprioception observed among young athletes diagnosed with ankle lateral ligament injury.

Abstract: Isometric eversion and inversion testing using the Biodex 4 Pro system is a reliable method. The authors suggest that the angle of 7° of inversion is the best for isometric eversion and inversion testing.
