Purpose To report the long-term sequential morphological and functional results in eyes with metamorphopsia after retinal detachment (RD) repair. Patients and methods In six eyes of six patients aged 58.7±11.0 years with metamorphopsia after successful buckling surgery for macula-off RD, best-corrected visual acuity (BCVA), fundus biomicroscopy, Amsler grid test, time-domain optical coherence tomography (TD-OCT) and central 121 microperimetry (MP-1) were performed at months 1, 3, 6, 12, and 18. At 5 to 6 years after surgery all patients underwent also spectral domain (SD)-OCT. Results Three eyes slowly recovered pre-RD BCVA. In the remaining three eyesFwith good final BCVAFthe interrupted junction line between photoreceptor cell inner and outer segments (IS/OS) was progressively less evident after RD surgery; and the external limiting membrane was preserved on SD-OCT examination. In all eyes post-operative metamorphopsia faded with time, but fully disappeared in 6 years only in two eyes without photoreceptor abnormalities. One of the two eyes with subretinal fluid up to 6 months and IS/OS disruption had central dense scotoma with relatively unstable fixation on MP-1 and persistent metamorphopsia. Macular sensitivity (MS) increased from 9.7 ± 7.1 at month 1 to 13.5 ± 5.6 dB at the final check, and was weakly (r ¼ 0.33) correlated with post-operative BCVA and OCT abnormalities. Conclusion Long-standing metamorphopsia can occur after successful macula-off RD repair even without detectable photoreceptor disruption on OCT. Post-operative BCVA recovery weakly correlates with increasing MS, and late restoration of the photoreceptor layer may be observed

Purpose: To determine and compare the intersession test-retest variability of different microperimetric parameters in healthy volunteers and age-related macular degeneration (AMD) patients by MAIA microperimetry.Methods: Twenty-four eyes of 12 healthy volunteers and 22 eyes of 11 AMD patients were included in this study. Microperimetry examinations were performed three times on each eye of all participants by MAIA microperimetry (CenterVue, Padova, Italy). The second measurement was performed on the same day and the third measurement one week after the first test. Retinal light sensitivity, stability of fixation, and macular integrity were recorded. Statistical analysis was performed by Microsoft Excel software and StatSoft Statistica software.Results: During the examinations, the average threshold had not changed significantly (p>0.475) in time in healthy and AMD patients. Fixation stability was relatively constant (nearly 100%) in healthy subjects as compared to the increase in the improving eye condition of AMD patients (p=0.042). Macular integrity was stable in both groups during the examinations. Conclusion:A learning effect was found in fixation stability of AMD patients by MAIA microperimetry. The improvement in fixation stability might be considered, especially prior to biofeedback training protocols in AMD patients.

The Microperimetry of Resolved Cotton-Wool Spots in Eyes of Patients With Hypertension and Diabetes Mellitus

Background: Retinal cotton-wool spots (CWSs) are an important manifestation of retinovascular disease in hypertension (HTN) and diabetes mellitus (DM). Conventional automated perimetry data have suggested relative scotomas in resolved CWSs; however, this has not been well delineated using microperimetry. This study evaluates the retinal sensitivity in documented resolved CWSs using microperimetry. Methods: Retinal CWSs that resolved after 10 to 119 months (median, 51 months) and normal control areas were photographed to document baseline lesions. Eyetracking, image-stabilized microperimetry with simultaneous scanning laser ophthalmoscopy was performed over resolved CWSs, adjacent uninvolved areas near the lesion, and in location-matched normal patients (age-matched). Results: A total of 16 eyes in patients with DM or HTN (34 resolved CWSs) and 16 normal control eyes (34 areas) were imaged. The mean (SD) sensitivity of resolved CWSs in the eyes of patients with HTN and DM was 11.67 (3.88) dB and 7.21 (5.48) dB, respectively. For adjacent control areas in the eyes of patients with HTN and DM, the mean (SD) sensitivity was 14.00 (2.89) dB and 11.80 (3.45) dB, respectively. Retinal sensitivity was significantly lower in areas of resolved CWSs than in the surrounding controls for patients with HTN (P = .01) and those with DM (PϽ.001). Scotomas in patients with DM were denser than those of patients with HTN (PϽ.05). Conclusions: Cotton-wool spots in patients with DM and HTN leave permanent relative scotomas detected by microperimetry. Scotomas are denser in eyes of patients with DM than in those with HTN. In addition, among patients with DM, adjacent retinas not involved with CWSs have lower retinal sensitivity than in age-matched controls.
