Purpose To determine the association between frailty, 30-day complications, rehospitalization, and transplant-free survival (TFS) following conventional and drug-eluting bead transarterial chemoembolization. Materials and Methods A retrospective analysis was performed on a cohort of 125 patients with treatment-naïve hepatocellular carcinoma who underwent conventional or drug-eluting beads chemoembolization at our institution between 2014 and 2015. Liver function parameters, Barcelona clinic liver cancer tumor stage, and all components of the five-item modified frailty index (mFI-5) were used to determine the patient's frailty status. Key end points included severe (grade 3 or above) adverse events of chemoembolization, 30-day rehospitalization rates, and TFS. Logistic regression analysis was performed on conventional predictors of postoperative complications after chemoembolization. Median survival was estimated and compared using the Kaplan–Meier's estimator and log-rank test. Results Among 125 patients who underwent first-time chemoembolization, higher frailty score was an independent predictor of both 30-day hospital readmission and severe liver toxicity (p = 0.01 and p = 0.03, respectively) on multivariate logistic regression analysis. Each point increase in mFI-5 conferred a threefold or twofold increase in the risk of experiencing 30-day rehospitalization or postoperative severe adverse events, respectively. At the data censor date, patients with mFI-5 score ≥ 2 had decreased overall TFS (28.1 vs. 39.8 months, p = 0.03). Conclusion Increasing frailty as determined by mFI-5 is an independent predictor of 30-day complications and lower TFS following chemoembolization.

Air pollution emanating from particulate matter exposure has been identified as a major environmental problem worldwide. The stone quarry industry generates enormous amount of particulate matter into the atmosphere because of activities like blasting, crushing and haulage. The study determined the levels of PM 10 and TSP concentrations (using IOM dust samplers) as well as lung function indicators of inhabitants of the Consar Stone Quarry concessional area from November 2015 to March 2016. PM 10 concentration exceeded the WHO guideline value (50µgm-3) and the permissible EPA-Ghana standard of 70µgm-3 except in the month of November and March. The measured TSP concentrations were generally below the EPA Ghana standard of 230µgm-3 except for January and December that recorded concentrations above the EPA-Ghana standard probably due to the low relative humidity resulting from scarcity in rainfall and high atmospheric temperature. Measured lung function indicators were significantly higher than the predicted values suggesting adverse effect of the quarry on inhabitants of the concessional area.

Derleme Yaşlanma, biyomoleküler düzeyde yapısal ve fonksiyonel bozulmayla birlikte kanser, nörodejeneratif bozukluklar, sarkopeni ve karaciğer yetmezliği gibi birçok patolojik durumu içeren karmaşık bir süreçtir. Yaşlanma süreci vücut sistemlerinin işleyişini değiştirdiğinden (kardiyorespiratuar, kas iskelet sistemi, sinir sistemi, bağışıklık sistemi, endokrin sistem v.b.) yaşlı insanlarda fonksiyon bozuklukları ve yetersizlikler görülür. Yaşlanma sürecinde görülen kalp yetersizliği, diyabet, osteoporoz gibi hastalıklar daha çok hareketsizliğin sonucudur. İleri yaşlara kadar aktif kalmak ve bağımsız yaşam için fonksiyonel kapasiteyi arttırmak gerekir. Bu nedenle bağımsız bir yaşamın sürdürülmesinde yaşam boyu fiziksel aktivite önemlidir. Fiziksel aktivite sadece sağlıklı yaşlılarda değil kronik hastalığı olan yaşlılarda da son derece yararlıdır. Bu derleme, yaşlıların hangi tip egzersizleri yapması gerektiğini ve egzersizin yaşlanma süreci üzerine etkilerini çeşitli çalışmalar ışığında açıklamaya çalışacaktır.
