The Benefits of Using a Small Caliber Ureteroscope in Evaluation and Management of Urethral Stricture

Abstract: Introduction and Objective The proper evaluation of urethral strictures is an essential part of the surgical planning in urethral reconstruction. The proper evaluation of the stricture can be challenging in certain situations, especially when the meatus is involved. We propose that the use of a small caliber ureteroscope (4.5 Fr and 6.5 Fr) can offer additional help and use for the surgical planning in urethroplasty. Methods We prospectively collected data on 76 patients who underwent urethroplasties in Kulkar… Show more

“…If the stricture is too narrow to allow passage of the cystoscope, no further information about the proximal urethra can be obtained with this diagnostic modality. In these cases it can be helpful to introduce a smaller caliber ureteroscope (4.5 or 6 Fr) which is able to pass through the strictured area and which thus provides further proximal information [14]. Apart from that, urethroscopy is also unable to provide any information about the surrounding spongiofibrosis.…”

“…Noteworthy, urethrocystoscopy and CUG/VCUG have been considered the preferred tools in post-urethroplasty follow-up protocols to detect a recurrent stricture [ 15 , 16 ]. Yet, urethroscopy rarely enables assessment of the stricture length as the caliber of symptomatic strictures is usually narrower than the standard cystoscopes used [ 17 ]. Moreover, urethroscopy is limited in providing a clear diagnosis in complex cases such as multiple strictures, or complete urethral obliteration.…”

“…An IPG was considered for patients with poor oral hygiene. A near-obliterated BUS was defined as a urethral caliber <6F on urethroscopy [7] . The choice between both approaches was intraoperative based on (1) landmark identification (the relation between the bulbospongiosus muscle, and the distal end of the urethral stricture) and (2) corpus spongiosum width.…”
