The Bentall procedure: Is it the gold standard? A series of 597 consecutive cases

Abstract: The choice of valve for aortic root reconstruction seems to have no influence on long-term outcome. Emergency operation and the presence of clot/atheroma have a significant impact on short-term outcome. Reoperation for either ascending aorta and/or aortic valve is low.

“…On 5, 10, and 15 years actuarial composite valve graft-related event-free survival was 97%, 82% and 54%, respectively (4-7). There was no difference in long-term survival between bioprosthetic and mechanical valves (8). All-cause mortality and valve-related complications were not significantly different between these two types of prostheses (9).…”

“…There were no bleeding events in our series. Valve-related complications after VSRR are fewer compared with Bentall procedure which, in the literature, arose with a frequency of 13% for reoperation, 7% for nonstructural dysfunction, 4-18% for thromboembolic events, 12% for endocarditis, 9-13% for bleeding events (5,8,9,19).…”

“…This procedure includes the replacement of the aortic valve, aortic root and ascending aorta with the reimplantation of the coronary arteries [110]. Excellent long-term results have been observed with the Bentall procedure with respect to survival in patients with aortic valve disease and aorta dilatation [111]. This procedure is a suitable intervention in older patients with severe BAV pathology with normal-sized sinuses and dilatation of the supracoronary ascending aorta.…”
