The Buccal Flap—A Useful Technique in Cleft Palate Repair?

Abstract: The technique presented has been effective, with the advantages of palatal closure without tension, good muscular reconstruction, lengthening of the nasal layer, and palatal closure without raw areas. The technique, early repair, and surgeon's skills were the most important variables for good outcomes regarding speech and fistula formation.

“…BFP is another reliable pedicled flap for oral reconstruction; 10,11 and has been previously employed in cleft palate surgery. 12 It is noteworthy to mention that the transverse and antero-posterior dimension of the maxilla of adults with unrepaired cleft palate in the present study was near to normal. However, this was at the expense of normal speech; and most of these patients presented with either hyper/hyponasality.…”

“…It should lengthen the palate and reconstruct the levator muscular sling to allow an efficient velopharyngeal valving action during speech, thus establishing conditions for good velopharyngeal closure [7]. Hence it is a challenge to repair the soft palate both structurally and functionally [8].…”

“…Unless the cleft surgeon reconstructs a palate that exhibits adequate mobility, normal speech will not be produced. So it is a challenge to repair soft palate structurally and functionally [7]. The three most common procedures used by surgeons to correct structural defects of VPI include Vol.…”

“…We adopt a modified V-Y palatoplasty for cleft palatal repair, although a large number of surgeons have developed surgical procedures for palatal repair [3][4][5][6][7][8][9][10]. The reason why we adopt a modified V-Y palatoplasty for cleft palatal repair is due to the following previous reports.…”
