The Causes of Chest Pain in Children and the Criteria for Targeted Myocardial Enzyme Testing in Identifying the Causes of Chest Pain in Children

Abstract: Aims: Chest pain is a common complaint at pediatric cardiology clinics and often leads to an extensive cardiac evaluation. In this study, we analyzed the causes of chest pain in Chinese children and developed diagnostic procedures and criteria for targeted myocardial enzyme testing. Methods and Results: We retrospectively analyzed the clinical data of patients aged below 18 years visiting our hospital for chest pain between 2005 and 2019. Based on auxiliary exams and clinical diagnosis, we developed diagnostic … Show more

“…Chest pain is a common symptom in children, and the visit rate of chest pain as the first complaint in pediatric outpatient is gradually increasing ( 10 , 18 ). Since there is not enough relevant knowledge about the common causes of children’s chest pain, most parents believe that their child’s chest pain should be caused by cardiac diseases and may be life-threatening.…”

“…Until now, children with myocarditis, pericarditis, large ventricular septal defect, anomalous origin of the coronary artery, cardiomyopathy, pulmonary arterial hypertension, aortic dissection, moderate or severe left ventricular outflow tract obstruction, pulmonary embolism or arrhythmia were thought to have cardiac chest pain ( 1 5 , 11 16 ). According to our previous study, we enrolled suspected myocarditis as cardiac chest pain in children ( 10 , 17 ). Based on criteria that were applied in retrospect, chest pain was divided into cardiac or non-cardiac ( 1 5 , 13 – 17 ).…”

“…Cardiac causes were excluded in the present study, which have been considered the main causes by parents and pediatricians. Chen et al [23] reported on 7,251 children with chest pain and only 8.0% was of cardiac origin. Possible respiratory-system problems were also excluded, such as pharyngitis, allergic rhinitis, adenoidal or tonsillar hypertrophy, pneumonia, pleura effusion, and bronchiolitis obliterans.…”

“…On the other hand, non-emergent cardiac factors contributing to chest pain encompass valvular, infectious, or inflammatory conditions. Interestingly, a Study by Chen L et al aimed to investigate the causes of chest pain in Chinese children and found that causes of chest pain in 7,251 children who visited the hospital during a 15-year period; 53.0% of the cases were idiopathic, 29.1% were related to Musculoskeletal diseases, 9.1% were related to respiratory diseases, 8.0% were cardiac, 0.6% were related to gastrointestinal diseases, 0.16% were related to mental diseases, and 0.04% were related to other conditions [ 1 ]. Nevertheless, according to this study, idiopathic chest pain was the most common cause of chest pain [ 1 ].…”
