The Clinical Correlations between Diabetes, Cigarette Smoking and Obesity on Intervertebral Degenerative Disc Disease of the Lumbar Spine

Abstract: Study DesignRetrospective analysis of a nationwide private insurance database. Chi-square analysis and linear regression models were utilized for outcome measures.PurposeThe purpose of this study was to investigate any relationship between lumbar degenerative disc disease, diabetes, obesity and smoking tobacco.Overview of LiteratureDiabetes, obesity, and smoking tobacco are comorbid conditions known to individually have effect on degenerative disc disease. Most studies have only been on a small populous scale.… Show more

“…Many evidences showed that diabetes mellitus (DM) is a dependent risk factor in the progression of IVDD . Previous epidemiological investigations have reported that DM patients have a higher incidence of IVDD than the patients without DM, and that DM patients with IVDD are also much younger than non‐DM patients . It has been demonstrated that high glucose can accelerate autophagy, apoptosis, and senescence of adult rat NP cells in a dose‐ and time‐dependent manner .…”

“…Consequently, it is a clinical priority to identify ways to reduce IVD damage. Risk factors for IVD degeneration are multifactorial, and recent studies have shown that Diabetes Mellitus (DM), obesity or being overweight add significant morbidity to back pain and IVD degeneration (Liuke et al, 2005, Takatalo et al, 2013, Jakoi et al, 2017, Fabiane et al, 2016 as they increase the risk for herniation (Sakellaridis, 2006), spinal stenosis (Anekstein et al, 2010) and spinal surgery complications (Guzman et al, 2014). DM and obesity are increasing at alarming rates, making this a growing high-risk population.…”

“…Smoking, obesity and diabetes mellitus are positively correlated with DDD. Of the 3 factors, smoking is the highest correlations to DDD and their effect is synergistic [62]. ADAMTS5 is the primary aggrecanase found in animal study that mediating smoking-induced disc aggrecanolysis and DDD [63].…”
