The contribution of diffusion-weighted MR imaging to distinguishing typical from atypical meningiomas

Abstract: Diffusion-weighted MR imaging findings of atypical/malignant meningiomas and typical meningiomas differ. Atypical/malignant meningiomas have lower intratumoral ADC values than typical meningiomas. Mean ADC values for peritumoral edema do not differ between typical and atypical meningiomas.

“…They are more common in older age and in females [1]. Although they are generally benign tumors, up to 10% of meningiomas are atypical or malignant [3]. MRI is a key modality not only for lesion diagnosis, but also to evaluate the extension, type and grade of the tumor.…”

“…DWI plays no role in the diagnosis of typical meningiomas and indeed can be somewhat variable [43]. DWI may, however, be useful to differentiate typical meningiomas from atypical and malignant meningiomas that account for approximately 9.6% of all meningiomas and are much more likely to recur [43].…”

“…DWI may, however, be useful to differentiate typical meningiomas from atypical and malignant meningiomas that account for approximately 9.6% of all meningiomas and are much more likely to recur [43]. Making the distinction with conventional MRI alone can be challenging because worrisome features such as heterogeneity, perilesional oedema, irregular surface and invasion of adjacent structures can also be seen in typical meningiomas [44].…”
