The difficult airway with recommendations for management – Part 2 – The anticipated difficult airway

Abstract: BackgroundAppropriate planning is crucial to avoid morbidity and mortality when difficulty is anticipated with airway management. Many guidelines developed by national societies have focused on management of difficulty encountered in the unconscious patient; however, little guidance appears in the literature on how best to approach the patient with an anticipated difficult airway.MethodsTo review this and other subjects, the Canadian Airway Focus Group (CAFG) was re-formed. With representation from anesthesiol… Show more

“…It is well known that LMAD insertion can be difficult in patients with fixed flexion of the cervical spine. [13][14][15] This has been attributed to the relatively acute angle between the oral and pharyngeal axes. 13 Nevertheless, it may be reasonable to attempt to insert a supraglottic airway to restore airway patency in an emergency before release of the halo.…”

“…In this issue of the Journal, the Group proposes a set of recommendations designed for two different contexts, the unanticipated difficult intubation 2 and the anticipated difficult airway. 3 Contrary to what could have been expected, the Group did not recommend a given device or a specific method when approaching the difficult airway, which will probably be a disappointment for those who like simple recipes. In spite of, and probably because of, the multiplicity of new tools and approaches, safe airway management is not simpler than it was in the past.…”
