The EDSS integration with the Brief International Cognitive Assessment for Multiple Sclerosis and orientation tests

Abstract: The use of brief neuropsychological tests leads to a more accurate CFS assessment in two-thirds of MS patients, and a more accurate EDSS calculation in 25% of patients with a score ⩽4.0. This may help clinicians to better recognize cognitive impairment in everyday clinical practice, especially in the case of isolated cognitive worsening.

“…Another advantage of using the BICAMS battery for assessing brain function FS is its administration time: it requires less than 15 minutes. Similar to the study by Saccà et al, 7 inclusion of the modified brain function FS did not affect the final EDSS score when scores were higher than 4.0. As previously stated, this is the result of the effect that gait alterations have on scores equal to or higher than 5.…”

“…The brain function FS was redefined in the EDSS by a new scoring system based on a change in assessment proposed by Saccà et al 7 The "basal EDSS" score (which excludes brain function) and the "modified EDSS" score, proposed for this work, were calculated. To calculate the modified EDSS score, the scores obtained in the tests of the BICAMS battery 8 were used ( Table 1).…”

“…[8][9][10][11][12][13] Despite this, cognitive issues are not adequately addressed, as traditional neurologic practice dominates over interdisciplinary approaches. 2,7,14 Up to 50% of people with MS experience depression. [14][15][16] Other mental health concerns (anxiety, psychosis, bipolar disorder, suicide, etc [17][18][19] ) are also prevalent and negatively affect HRQOL.…”

“…3 Cognitive impairment (present in up to 70% of people with MS 4 ) leads to a greater likelihood of psychosocial challenges 5,6 and disability. 7 Methods exist to assess and instead relied on observation of overt signs of cognitive impairment, which is known to be inadequate. 14 No survey looked at psychosocial concerns.…”

“…Their results suggests, that a quarter of patients with EDSS≤4 points without integration of BICAMS is miscalculated and the disability underestimated (Sacca et al, 2017). Furthermore, in 2018, a guideline was proposed for the measurement of cognition in PwMS (Kalb et al, 2018).…”
