The effect of biotin interference on the results of blood hormone assays

Abstract: Approximately 70% of medical decisions are made based on results of laboratory investigations. Immunochemical methods are used most commonly in routine laboratory diagnostics of endocrine disorders. Those methods are limited by susceptibility of the immunochemical reaction to various interferences. Interference may be caused by the presence of autologous antibodies, heterophilic antibodies, or paraproteins in the blood serum, by cross-reactions with similar reagents, haemolysis, significant lipidaemia, or hype… Show more

“…doi: 10.5858/arpa.2019-0425-OA) L aboratory analyses play a paramount role in about 70% of medical decisions and must be highly reliable to avoid false or missed diagnoses or unnecessary diagnostic procedures and treatments. 1 Currently, around 85% of the most common immunochemical analyzers are equipped with assays using the interaction between streptavidin and biotin (vitamin H/B 7 ; coenzyme R). The design offers advantages including signal amplification and increased sensitivity.…”

“…The design offers advantages including signal amplification and increased sensitivity. [1][2][3][4] Unfortunately, this system can be vulnerable to interference from endogenous high biotin concentrations, causing false low results in sandwich immunoassays and false high results in competitive immunoassays. The skewed results can lead to misdiagnoses including suspected endocrine and autoimmune disorders, missed infectious or neoplastic diseases, delayed pregnancy detection, suspected drug intoxications, and missed myocardial infarctions.…”
