Abstract: We conclude that the routine use of preoperative physiotherapy and education programme is not useful in total hip replacement surgery.

“…Of the 36 interventions (32 articles) targeting optimisation of pre-surgical health, 14 interventions were in patients waiting for THR, [487][488][489][493][494][495][496][497][500][501][502][503][504]507 17 in patients waiting for TKR, 500,503,509-515,517,519-521,523-525 and five in total hip and knee patients together. [526][527][528][529]534 Eighteen interventions were specifically exercise based, 488,494,497,502,503,509,[511][512][513][514][515]519,520,[523][524][525] nine were education based, 52,489,495,500,501,526,527,529 while nine were multifactorial with exercise and education components and, in some cases, an occupational therapy base.…”

“…The exercise group had improvement in all variables 1 week before and up to 6 months after surgery [18]. Gocen et al [20] reported a prospective randomized study of 60 patients with osteoarthritis undergoing THA. Subjects in the study group received preoperative physiotherapy designed to strengthen the muscles of the upper and lower limbs and to improve range of motion of the hip, beginning 8 weeks before the operation.…”

“…In their sample of 31 patients, few differences were observed between the patients and control subjects on measures such as the visual analogue pain scale and the Harris hip score (Wijgman et al 1994). More recent work by Gocen et al (Gocen et al 2004) also showed that instruction and pre-operative physiotherapy is of no benefit in terms of improving outcome (measured with the Harris Hip Score and Visual analogue scale) after THR surgery. A systematic review by Ackerman and Bennell (Ackerman & Bennell 2004) found that only two randomised controlled trials involving patients undergoing THR surgery demonstrated a benefit of performing pre-operative exercise.…”
