The effect of proton pump inhibitors on fracture risk: report from the Canadian Multicenter Osteoporosis Study

Abstract: In this large prospective population-based cohort study, we found an association between PPI use and increased risk of fragility fracture. Although the increased risk found was modest, this finding is important, given the high prevalence of PPI use and the excess morbidity and mortality associated with osteoporosis-related fractures.

“…In sensitivity analysis, for participants who interrupted drug use during a given 5-year period, time-dependent exposure was defined as a variable [12] whose time-dependent value gradually decreased from initial value of 1 (in the first year of the period) to 0 in the last year, in equal steps of 0.2 reduction for each additional year. For example, a participant who reported use in year 0 but no use in year 5 was assigned values of 0.8, 0.6, 0.4, and 0.2, respectively, for years 1-4.…”

“…5C). There have also been reports showing that some drugs (e.g., steroids, TZD, and PPIs) and comorbidities (e.g., SLE, RA, DM or CKD) are associated with the occurrence of insufficiency fractures [10][11][12]. These factors are also considered to be involved in the development of AFF.…”

“…The discontinuance of PPIs in many cases causes the patients to be affected by increased acid secretion and recurrence of symptoms (14,15). hip fractures (18), reduced absorption of nutrients such as calcium and magnesium (19), kidney problems (20), and dementia (21). Therefore, if we could reduce the reversibility and duration of PPI treatment, we hope to successfully treat reflux.…”
