Endogenous steroid hormones, especially glucocorticoids and mineralocorticoids, derive from the adrenal cortex, and drastic or sustained changes in their circulatory levels affect multiple organ systems. Although hypoxia signaling in steroidogenesis has been suggested, knowledge on the true impact of the HIFs (Hypoxia-Inducible Factors) in the adrenocortical cells of vertebrates is scant. By creating a unique set of transgenic mouse lines, we reveal a prominent role for HIF1α in the synthesis of virtually all steroids in vivo. Specifically, mice deficient in HIF1α in adrenocortical cells displayed enhanced levels of enzymes responsible for steroidogenesis and a cognate increase in circulatory steroid levels. These changes resulted in cytokine alterations and changes in the profile of circulatory mature hematopoietic cells. Conversely, HIF1α overexpression resulted in the opposite phenotype of insufficient steroid production due to impaired transcription of necessary enzymes. Based on these results, we propose HIF1α to be a vital regulator of steroidogenesis as its modulation in adrenocortical cells dramatically impacts hormone synthesis with systemic consequences. In addition, these mice can have potential clinical significances as they may serve as essential tools to understand the pathophysiology of hormone modulations in a number of diseases associated with metabolic syndrome, auto-immunity or even cancer.

Most of the clinical features of Cushing’s syndrome (CS) are nonspecific and could be present in obesity, particularly when this condition is associated with metabolic syndrome. Our objective was to evaluate the frequency of clinical manifestations and changes in general laboratory tests, in patients with confirmed endogenous CS, to identify diagnostic dyads. We evaluated in each patient the rate of coexistence of 2 elements either: symptoms, clinical signs, or laboratory alteration. The prevalence of a combined pair of clinical features or dyad in over 30% of the cases, was considered clinically significant. Fourteen dyads were identified as clinically relevant. Facies + buffalo hump; facies + eosinopenia; buffalo hump + supraclavicular fat pads and facies + supraclavicular fat pads, were present in over 50% of cases. Facies + muscular atrophy; centripetal fat distribution + muscular atrophy and facies + striae were present in 42–49%. Hirsutism/acne + eosinopenia; buffalo hump + eosinopenia; muscular atrophy+ eosinopenia; eosinopenia + accelerated weight gain; buffalo hump + muscular atrophy; hirsutism/acne + muscular atrophy and hirsutism/acne + supraclavicular fat pads, were observed in 33–38% of patients. Its application will facilitate the correct diagnosis of CS.

Stress leukogram includes eosinopenia as one of its main markers (neutrophilia, eosinopenia, lymphopenia, and mild monocytosis). Cortisol is the main stress biomarker, which is also strongly correlated with the severity of gastrointestinal diseases. This study aimed to determine the relationship between salivary cortisol and the eosinophil cell count (EC) in equids with abdominal pain. To do this, 39 horses with abdominal pain referred to an emergency service were included. All samples were taken on admission, and several parameters and clinical data were included. Equids were classified according to the outcome as survivors and non-survivors. Non-surviving equids presented higher salivary cortisol concentrations (Non-Survivors: 1.580 ± 0.816 µg/dL; Survivors 0.988 ± 0.653 µg/dL; p < 0.05) and lower EC (Non-Survivors: 0.0000 × 103/µL (0.000/0.0075); Survivors: 0.0450 × 103/µL (0.010/0.1825); p < 0.01). In addition, the relationship between salivary cortisol concentration, EC, and the WBC was determined. Only a strong correlation (negative) was observed between cortisol and EC (r = −0.523, p < 0.01). Since cortisol is not an analyte that can be measured routinely in clinical settings such as emergencies, the EC could be a good alternative. While the results are promising, further studies are needed before EC can be used confidently in routine practice to predict survival in cases of abdominal pain.
