Objective:The aim of this study was to evaluate the effect of a totally implantable active middle ear implant in quality of life of the implanted patients taking into account their hearing performance. Methods:Retrospective observational nonrandomized group study. Settings: Private hospital. From March 2018 and September 2019, all the participant with minimum 6 months' experience with the Carina system were evaluated. The evaluation was performed using the Abbreviated Profile of Hearing Aid Benefit (APHAB) questionnaire.Results: Nine patients complete the entire questionnaire. A reduction of difficulties was seen in quiet environments/ease of communication (32.8% vs 54.3%), in background noise environments (42.8% vs 62.3%) and in environments with reverberation (38.7% vs 63.9%) in the patient with the active middle ear implant when compared to the unaided condition. In what concerned the trouble caused by unpleasant sounds/Aversiveness, an increased discomfort was described with de middle ear implant compared to the unaided condition (50.3% vs 32.8%). Conclusion:Active middle ear implants seem to improve auditory perception in daily activities, even in difficult conditions like background noise or reverberation environments, although statistically significant conclusions cannot be achieved due to the small sample size. APHAB demonstrated to be a potential useful tool in this evaluation.

Purpose To evaluate the long-term audiological outcomes and safety of the latest generation of middle ear transducer (MET) among a group of Polish patients. Methods Ten patients aged 48–72 years with bilateral sensorineural hearing loss (n = 8) and mixed hearing loss (n = 2) were included in this study. Pure tone audiometry, sound thresholds, word recognition scores in quiet and speech reception thresholds in noise were assessed. Medical and technical complication information was gathered. Results All the patients underwent unilateral implantation with the latest generation Cochlear MET between 2014 and 2016. Mean length of follow-up was 3.7 years. Compared to the unaided condition, the implant provided significant functional gain (mean M = 26.1 dB) at 12 months follow-up. Compared to before surgery, average word recognition in quiet at 65 dB and at 80 dB SPL, as well as speech reception threshold in noise, were significantly better at 12 months. However, postoperative air conduction thresholds 6 months after implantation worsened by 10.3 dB (standard deviation SD = 5.8 dB). Postoperatively, three patients had skin problems around the processor, and one of them completely resigned from using the device 5 months after activation. Technical failures occurred in 4 cases. There were 9 out of 10 patients who still used the MET, but only 5 of them used the processor regularly (every day). Conclusion Despite changes in the transducer implemented by the manufacturer, we observed a significant number of adverse events in users of the latest generation of MET.

ZusammenfassungDie Versorgung mit implantierbaren Hörgeräten stellt einen Bereich mit hohem Entwicklungs- und Innovationspotenzial dar. Der vorliegende Übersichtsartikel gibt einerseits einen Überblick über derzeitige Indikationskriterien für die Versorgung mit aktiven Mittelohrimplantaten. Andererseits werden Zielparameter sowie Ergebnisse nach der Versorgung mit aktiven Mittelohrimplantaten beleuchtet. Hierbei liegt der Fokus auf audiologischen Ergebnissen sowie dem vom Patienten bewerteten Gesundheitszustand. Sogenannte „Patient Reported Outcome Measures“ (PROMs) nehmen mittlerweile auch in der Hörimplantatversorgung einen festen Stellenwert in der Evaluierung des Versorgungserfolges ein. Bisher ist die Studienlage sowohl hinsichtlich audiologischer als auch subjektiver Erfolgsparameter gemessen an den Evidenz-Level-Kriterien nicht befriedigend. Ein bislang fehlender internationaler Konsens über akzeptierte Outcomeparameter erschwert eine metaanalytische Aufarbeitung der Ergebnisse immens. In den bislang publizierten Untersuchungen konnte sowohl für Patienten mit einer Schallempfindungsschwerhörigkeit als auch für Patienten mit einer Schallleitungs- oder kombinierten Schwerhörigkeit ein besseres Sprachverstehen mit dem aktiven Mittelohrimplantat im Vergleich zur konventionellen Hörsystemversorgung ermittelt werden. Aktuelle Analysen zeigen eine signifikante Verbesserung der allgemeinen sowie der hörspezifischen Lebensqualität nach Versorgung mit einem aktiven Mittelohrimplantat. Zur Beurteilung des Versorgungserfolges bei Kindern existieren bislang keine validierten, hörspezifischen Lebensqualitätsmessinstrumente. Insbesondere bei Kindern mit komplexen Fehlbildungen des äußeren Ohres sowie des Mittelohres zeigen sich audiologisch zufriedenstellende Ergebnisse, welche zukünftig jedoch durch Lebensqualitätsmessungen untermauert werden müssen.
