The “Hay-fork” sign in the ultrasonographic diagnosis of intussuception

Abstract: Ultrasonography was used to evaluate a palpable abdominal mass in a woman. The characteristic "hay-fork" image led to the correct diagnosis of colocolic intussusception, which had not been suspected. This sign appears to be highly specific for intussusception.

“…4 In previous studies, a doughnut sign and a pseudokidney sign have been used as diagnostic criteria for intussusception, 21 although these signs are now considered not to be pathognomonic as they may also be noted for some other conditions as well, including inflammatory or edematous bowel-wall thickening, hematoma, enterocolitis, volvulus, and even stool impaction in the colon. 3,4,[6][7][8][9][21][22][23][24][25] Other typical characteristic sonographic features of intussusception have been described including a crescent-indoughnut sign and a multiple-concentric-rings sign on axial scanning or a sandwich sign and a hayfork sign on longitudinal scanning.…”

“…Intussusception also produces a characteristic appearance, with a focal mass as a lead point. The layers of the intussusceptum and the intussuscipiens show as a series of concentric rings in cross section and have been likened to a hay fork when seen in long axis [21]. Intraluminal intestinal masses causing obstruction, although rare, are also clearly seen on sonography.…”

“…The bowel wall may be edematous. Free peritoneal fluid may be identified, which does not necessarily indicate the presence of peritonitis [5][6][7]. The target sign is not specific for intussusception and may be seen with other disorders.…”
