BackgroundHome-based training is an alternative option to provide intensive rehabilitation without costly supervised therapy. Though several studies support the effectiveness of home-based rehabilitation in improving hemiparetic upper limb function in stroke survivors, a collective evaluation of the evidence remains scarce.ObjectivesThis study aims to determine the effects of home-based upper limb rehabilitation for hemiparetic upper limb recovery in stroke survivors.MethodsThe databases of the Cochrane Library, MEDLINE, CINAHL, and Web of Science were systematically searched from January 2000 to September 2020. Only randomized, controlled, and cross-over trials that evaluated the effects of home-based upper limb interventions were selected. The Pedro scale was used to assess the methodological quality of the studies. A meta-analysis of the upper limb function outcomes was performed by calculating the mean difference/standardized mean difference using a fixed/random effect model.ResultsAn initial search yielded 1,049 articles. Twenty-six articles were included in the review. The pooled evidence of the meta-analysis showed that home-based upper limb intervention was more effective in improving upper limb function [SMD: 0.28, 95% CI (0.12, 0.44), I2 = 0%, p < 0.001, fixed effect model] than conventional therapy. When comparing two types of home-based interventions, subgroup analysis revealed that home-based technology treatment—electrical stimulation—provided more significant improvement in upper limb function than treatment without the use of technology (SMD: 0.64, 95% CI (0.21, 1.07), I2 = 0%, p = 0.003, random effect model).ConclusionThe beneficial effects of home-based upper limb interventions were superior to conventional therapy in improving function and perceived use of the hemiparetic upper limb in daily activities. Among the home-based interventions, home-based electrical stimulation seemed to provide the most optimal benefits.

The purpose of this pilot study was to assess of home care intervention on post-stroke related outcome of range of motion and muscle strength. Sample in 40 participants were divided into the intervention group and control group and included in this study according to the following criteria: post-stroke period of <12 months with hemiparesis, age of ≥18 years, and willingness to participate in the study. The intervention was carried out by nurses by providing education for 2 consecutive days and mentoring for 5 consecutive days, while the control group was given standard care and measured using a grip track, handheld dynamometer, and goniometer examination on June to September 2019. For the intervention group, paired t-test analysis confirmed a significant increase in the mean upper extremity muscle strength before (35.770 ± 46.063) and after (51.073 ± 50.866) the 7 day intervention ( p = .002), whereas the control group showed a value 36.570 ± 33.684 and then 31.400 ± 31.760 p = .256 and lower extremity strength before (3.627 ± 1.585) and after (4.365 ± 1.698) the 7-days intervention ( p = .000), whereas the control group showed a value 3.657 ± 1.671 and then 4.043 ± 1.849 p = .013. Almost all the items assessed from Range of Motion (ROM) in the upper and lower extremities showed a significant increase ( p < .05). Supportive-educative ROM exercise significantly contributed to an increase in the average muscle strength and ROM in post-stroke patients.

É cada vez maior o número de pessoas que apresentam debilidade motora em membros superiores e enfrentam deficiências funcionais, dependendo de terceiros para realizar suas atividades de rotina. A reabilitação motora é caracterizada por uma rotina diária de deslocamento para as clínicas, exercícios físicos repetitivos e um lento processo de recuperação funcional, o que torna o tempo de tratamento muito longo, levando o paciente à falta de compromisso e abandono do tratamento. Nesse contexto, soluções de gameterapia têm sido utilizadas para a reabilitação motora, em que se utilizam cenários interativos de jogos sérios para potencializar a humanização do tratamento e a experiência do paciente durante a execução de exercícios de recuperação das funções motoras. Este artigo tem como objetivo apresentar um jogo sério inteligente para reabilitação motora de membros superiores, a partir do uso de modelos de visão computacional e aprendizado de máquina integrados a tecnologias com ampla inserção social, como câmeras embutidas em smartphones, laptops ou integradas em TVs. Para implementação do jogo sério, foram utilizadas as linguagens de programação web, HTML, CSS e JavaScript, incorporando a biblioteca TensorFlow e o pacote PoseNet para controlar os movimentos do paciente nos cenários do jogo. O jogo sério desenvolvido contribui para tornar a gameterapia uma técnica presente e viável em clínicas de reabilitação e ambientes domiciliares, uma vez que sua inserção não depende de custos associados com a tecnologia ou adaptações de infraestrutura no ambiente.
