The lateral and medial approach in total arthroplasty for valgus knee: a meta-analysis of current literature

Abstract: Aim: To compare the lateral and medial approaches of total knee arthroplasty (TKA) in the valgus knee. Materials & methods: An electronic search from the PubMed, Embase, Web of Science and Cochrane library was performed according to ‘TKA’, ‘valgus’, ‘knee’ and ‘approach’. Subsequently, manual search was conducted from the reference lists in the identified studies. Results: Four randomized controlled trials and five cohorts were included. Better knee society score and function was noticed in patients after … Show more

“…Although there are several randomized controlled trials comparing the MPA, MV, SV, and LPA approaches, many are small in sample size, report mixed results, and have short follow-up duration. 16,17 We aimed to add to this body of literature by reporting outcomes of a singlesurgeon, expertise-based study. The MV approach for TKA was first described as a modification of the SV approach to allow for easier expos ure.…”

“…A recent meta-analysis also compared the LPA to the MPA for valgus knees and found improved KSS in the LPA group, but similar alignment, operative duration, blood loss, WOMAC scores, postoperative pain, and ROM between the 2 approaches. 16 As the LPA is commonly described for valgus knees, allowing for direct access to the tight lateral structures, our analysis for this group involved only knees with valgus preoperative alignment. Our comparison of the MPA and LPA in valgus knees actually showed significantly lower SF-12 PCS (p = 0.011) and WOMAC function scores (p = 0.022) at 1 year postoperative for the LPA group.…”

“…The remainder of the issue includes a range of surgically related research. In their paper, Li et al compare lateral and medial approaches of total knee arthroplasty (TKA) in the valgus knee, a deformity found in around 10-15% of TKA patients [3]. Ni et al also compare surgical techniques; in their paper they look at the comparative clinical and radiographic outcomes of two types of minimally invasive surgery for degenerative spondylolisthesis: transforaminal lumbar interbody fusion and oblique lumbar interbody fusion [4].…”

“…Total knee arthroplasty (TKA) has become the most effective method for the treatment of end-stage knee joint diseases [1]. Valgus knee deformity (≥ 10°) is observed in nearly 10% of patients who underwent TKA [2]. Valgus knee deformity is a challenge in TKA because the soft tissue release and balancing are crucial for long-term success [3].…”
