The link between otitis media with effusion and allergy: A potential role for intranasal corticosteroids

Abstract: We reviewed the evidence linking otitis media with effusion (OME) and atopy, with the goal of clarifying the possible role of intranasal corticosteroids (INSs) in OME treatment. In August 2009, the MEDLINE database was searched for primary studies on OME epidemiology, pathophysiology, and treatment. Relevant clinical guidelines were obtained. Interpreting OME research is complicated by variable disease definitions, patient populations, methodologies, and outcomes assessments, along with the possibility of spon… Show more

“…Indoor allergens and respiratory allergies such as allergic rhinitis contribute to the onset of COM/ROM. The prevalence of atopic conditions, including allergic rhinitis in patients with COM/ROM ranges from 24% to 89% [41]. New evidence from cellular biology and immunology explained allergy as a cause for Eustachian tube (ET) obstruction [42].…”

“…Many risk factors have been associated with this disease: young age (6,7) lack of breast feeding (6,8,9) low mother education, (9)(10)(11) low socioeconomic status (6,10) day care attendance (8,11) parental smoking, (6,11,12) upper respiratory tract infections (6,9,11) allergy (6,9,11,13,14) and snoring. (6,9) Although it is a self-limiting condition in the majority of cases, OME may become chronic to the extent that it affects child education and quality of life.…”

“…Recent otolaryngology literature has also explored the role of allergies as the underlying cause of multiple aerodigestive disease. 27,28 Limitations This study can only be considered a pilot study as it was performed in a cohort whom CMPA was diagnosed and had positive response to elimination with respect to their gastrointestinal manifestations. Future clinical studies confirming the validity of the questionnaire in patients not previously diagnosed with CMPA will be needed.…”
