The Past, Present, and Future in Management of Small Renal Masses

Abstract: Management of small renal masses (SRMs) is currently evolving due to the increased incidence given the ubiquity of cross-sectional imaging. Diagnosing a mass in the early stages theoretically allows for high rates of cure but simultaneously risks overtreatment. New consensus guidelines and treatment modalities are changing frequently. The multitude of information currently available shall be summarized in this review. This summary will detail the historic surgical treatment of renal cell carcinoma with current… Show more

“…The widespread use of advanced imaging techniques including ultrasonography, computed tomography (CT), and magnetic resonance imaging for patients presenting with nonspecific abdominal symptoms contributes to the increasing incidence of SRMs. Surgical resection of these early‐stage, potentially malignant renal tumors also is on the rise . However, a significant percentage of SRMs removed by nephrectomy later are proven to be either benign neoplasms or low‐grade RCC cases.…”

“…Ablative therapies for SRM are an option for nephronsparing intervention and can be performed using an open or laparoscopic approach or percutaneously with or without general anaesthesia [5,24]. A systematic review comparing laparoscopic cryoablation (LCA) to laparoscopic PN (LPN) suggested that LCA had less perioperative complications but was associated with a higher risk of tumour progression [25].…”

“…A systematic review comparing laparoscopic cryoablation (LCA) to laparoscopic PN (LPN) suggested that LCA had less perioperative complications but was associated with a higher risk of tumour progression [25]. This is therefore generally an option for those with multiple comorbidities or elderly patient and is the chosen option after a careful discussion of risks and benefits discussed with the patient [5].…”
