The Preferred Option for Multinodular Goiter

Abstract: Total thyroidectomy is an operation that has generally been reserved for the management of differentiated thyroid carcinoma. Over the last decade total thyroidectomy has become used increasingly and is now the preferred option in the authors' unit for the management of multinodular goiter affecting the entire gland. Over the period from 1975 to 1985, 853 thyroidectomies have been performed for multinodular goiter; of these, 115 have been total thyroidectomies. During that time, the incidence of total thyroidec… Show more

“…The main objection about performing TT has been the higher likelihood of damage to the RLN and induction hypoparathyroidism [6]. However the incidences of permanent RLN injury and permanent hypoparathyroidism associated with thyroid reoperations have been reported to range between 3.1% and 9.5% and between 3.4% and 14% respectively [20,22]. Pappalardo et al reported a rate of recurrence up to 14.5% after subtotal resection in their study, even though pharmacological prophylaxis was given [4].…”

“…çalışmasında, 1975-1984 dö-neminde yapılan 789 tiroidektomi ameliyatında kardiyak nedenlerden kaynaklanan 2 postoperatif ölüm görüldüğünü, TT ameliyatının rutin hâle geldiği 1985 yılında yapılan 64 tiroidektomi ameliyatında ölüm görülmediğini bildirmiştir. 44 Çalışma-mızda hematom gelişen olgulardan 2'si havayolu obstrüksiyonu nedeniyle 1. ve 5. günlerde kaybedilmiş; postoperatuar dönemde hipoksik ansefalopati gelişen üçüncü olgu ise ameliyattan 10 ay sonra vefat etmiştir. Postoperatuar dönemde solunum sıkıntısı gelişmesi nedeniyle trakeostomi açılan bir olgunun da ameliyattan 41 gün sonra kanülü çıkarılırken kanül kenarından oluşan kanamaya bağlı gelişen hipoksi nedeniyle kaybedildiği gözlenmiştir.…”

“…Total thyroidectomy has been established as the preferred operation for a range of thyroid pathologies including thyroid cancer, multinodular goitre (MNG) and Graves' disease. [1][2][3] Postoperative hypocalcaemia is the most common complication observed in patients undergoing total thyroidectomy, with a reported incidence ranging from 1.6% to 50%. [4][5][6][7][8] Transient hypocalcaemia occurs in upto 50% of patients and permanent hypocalcaemia in 2%.…”
